Long-term use of supraphysiological doses may cause centripetal obesity, moon face, purple striae, thinning skin, muscle weakness, muscle atrophy, hypokalemia, edema, nausea, vomiting, hypertension, diabetes, acne, hirsutism, infection, pancreatitis, poor wound healing, osteoporosis, induction or aggravation of gastrointestinal ulcers, growth inhibition in children, and induction of mental symptoms. Long-term and excessive use in the eyes can cause high blood pressure, leading to optic nerve damage, visual field loss, posterior subcapsular cataract, and secondary fungal or viral infections.

Indications for glucocorticoids

① Diffuse connective tissue disease: It has a positive effect in diseases such as systemic lupus erythematosus, mixed connective tissue disease, polymyositis/dermatomyositis, vasculitis, etc., and can prolong the patient's life span and improve the quality of life.

② Rheumatoid arthritis: mainly used for patients with vasculitis, serositis and organ damage, as well as patients who cannot receive treatment with non-steroidal anti-inflammatory drugs. It has a good effect on Felty syndrome, senile rheumatoid arthritis, rheumatoid seronegative symmetrical synovitis with pitting edema syndrome, and adult Still's disease.

③Allergic diseases related to immunity.

④ Other arthritis accompanied by damage to important organs: such as Reiter syndrome with iritis and heart damage, rheumatoid arthritis with heart damage, etc.

⑤ Nephritis, chondritis, ophthalmitis, serositis, peripheral and central nervous system diseases related to immune factors.

(1) Mineralocorticoids: They are synthesized and secreted by the zona glomerulosa of the cortex, represented by aldosterone and deoxycorticosterone. They have a certain effect on water and salt metabolism, so they are called mineralocorticoids. Less commonly used in medicine.

(2) Glucocorticoids: They are synthesized and secreted by the zona fasciculata of the cortex. They mainly affect the metabolism of sugar and protein, and can counteract inflammatory responses. They have little effect on water and salt metabolism and are widely used in clinical practice. The main drug is represented by hydrocortisone, and the "corticosteroids" generally referred to refer to this category. Artificially synthesized glucocorticoids have stronger anti-inflammatory effects and less impact on water and salt metabolism than natural hormones, and therefore are more important in application. Commonly used drugs include prednisone, prednisolone and flumethasone.
