Causes

1. The otoliths may fall off from the utricle due to spontaneous degeneration of the utricle and deposit on the posterior ampullary ridge.

2. Inflammation or trauma can also induce this disease. For example, during viral labyrinthitis or chronic suppurative otitis media, white blood cells, macrophages or endothelial cells enter the endolymph; after brain trauma or stapes surgery, blood enters the endolymph, forming tangible particles that are deposited on the top of the ampulla ridge.

3. Pathological findings showed that the utricle, superior semicircular canal and lateral semicircular canal distributed by the anterior vestibular artery and superior vestibular nerve branches were degenerated, while the saccule and posterior semicircular canal were normal. Therefore, it is believed that hypertension, heart disease and insufficient blood supply to the vertebral basilar artery can cause embolism of the anterior vestibular artery, leading to degeneration and shedding of the otolith membrane and deposition on the ampullary ridge.

4. Gammaglobulinemia, globulin deposition on the ampulla crista, or alcohol thinning of the ampulla crista in alcoholics, can both produce this syndrome. When severely deaf children wear high-intensity (120-130dB) hearing aids, about half of them experience positional vertigo.
