What are the symptoms of prostate cancer?

(I) No subjective symptoms in the early stage

Prostate cancer has no early symptoms. Prostate hyperplasia occurs in the prostate gland, which will show symptoms of urination disorder. Cancer is more likely to form in the prostate gland, and symptoms appear later. When you feel symptoms, it is already in the late stage of cancer.

Because there are almost no symptoms in the early stages of prostate cancer, the presence of prostate hyperplasia can mask the occurrence and progression of cancer. In fact, many patients who undergo examinations and treatment for prostate hyperplasia are found to have cancer.

Generally, the main obvious symptom of late-stage prostate cancer is the appearance of lower urinary tract obstruction, or accompanied by hematuria and urinary retention. The most prominent symptom is pain. When there is bone metastasis, there is severe pain in the pelvis or lumbosacral region. The main physical signs are obvious manifestations of local nodules and lumps in the prostate. The texture is hard, it loses its normal elasticity and becomes fixed. The gland is hardened on all sides and the boundaries are unclear.

(II) Local manifestations

(1) Symptoms of urethral obstruction When the tumor enlarges and blocks the urinary tract, obstruction symptoms may occur, manifested as gradually worsening slow urination, frequent urination, urgent urination, interrupted urinary flow, incomplete urination, and difficulty urinating. Dysuria and hematuria are often late symptoms. Obstruction caused by localized lesions often occurs acutely and worsens continuously. This is because the external gland lesions invade the internal glands, causing the compliance of urination to decrease. Literature reports that about 40% of prostate cancer patients have acute urinary retention as the first symptom.

(2) The impact of local tumor spread on surrounding tissue structures When the lesion extensively invades the urethral membranous part, urinary incontinence may occur. When the capsule and the lymph nodes around the nerves are invaded, compression of the nerves may cause local pain. Compression of the sciatic nerve may cause radiating pain in the lower limbs. When the rectum is compressed, defecation may become difficult. When the tumor metastasizes along the lymph nodes and causes compression and obstruction of the ureter, there may be symptoms of low back pain and hydronephrosis. Oliguria and renal failure may occur in bilateral cases. Prostatic duct carcinoma and transitional cell carcinoma often present with painless hematuria accompanied by frequent urination and dysuria. When the tumor invades the seminal vesicle, there may be hematospermia.

The most obvious symptoms are frequent urination, urgency, difficulty urinating or urinary incontinence.
