Indications for dialysis

Dialysis is mainly due to the kidneys. Renal dialysis can effectively treat acute renal failure and drug poisoning. However, for renal dialysis, we should choose a regular large hospital for treatment, so that it can achieve the therapeutic effect. The indications for renal dialysis are mainly due to abnormal renal function, such as uremia and heart failure. You can learn about its treatment targets.

What are the indications for dialysis?

(1) Acute renal failure: ① Anuria or oliguria for more than 2 days (48 hours), accompanied by hypertension, water intoxication, pulmonary edema, or cerebral edema; ② BUN ≥ 35.7 mmol/L (100 mg/dl) or daily increase > 10.7 mmol/L (30 mg/dl); ③ Scr ≥ 530.4 μmol/L; ④ Hyperkalemia, K+ ≥ 6.5 mmol/L; ⑤ Metabolic acidosis, CO2-CP ≤ 13 mmol/L, and correction is ineffective.

(2) Chronic renal failure: ① Scr ≥ 884 μmol/L (10 mg/dl); ② BUN ≥ 35.7 mmol/L (100 mg/dl); ③ Ccr ≤ 5 mL/min. Those with the following conditions: ① Heart failure or uremic pericarditis; ② Uncontrollable hyperphosphatemia, soft tissue calcification found in clinical and X-ray examinations; ③ Severe electrolyte imbalance or metabolic acidosis, such as K+ ≥ 6.5mmol/L, CO2-CP ≤ 13mmol/L; ④ Obvious water and sodium retention, such as severe edema and high blood pressure; ⑤ Severe uremic symptoms, such as nausea, vomiting, fatigue, etc.

(3) Acute drug or poison poisoning: If the poison can be precipitated through the dialysis membrane and the dose is not large or the speed of action is not too fast, dialysis can be performed. It should be performed within 8 to 16 hours after taking the poison. Emergency dialysis should be performed in the following situations: ① After conventional treatment, the condition still worsens, such as coma, slow or absent reflexes, respiratory arrest, refractory hypotension, etc.; ② It is known that the poison has entered the body or the concentration of the poison in the blood has reached a lethal dose; ③ The organs that normally excrete the poison have significantly reduced function due to primary diseases or damage by the poison; ④ Combined lung or other infections.
