Six measures for nursing care of pulmonary encephalopathy

1. Mild laxative measures: clear out food or blood in the intestines (after gastrointestinal bleeding). Take 40 ml of 50% magnesium sulfate orally, or use high-level enema with sodium acetate buffer solution with pH>4.5. Families can use normal saline plus 50 grams of vinegar for cleansing enema.

2. Dietary treatment: When early symptoms of hepatic coma appear, stop eating protein-rich foods such as eggs, milk, and meat. The condition may be alleviated or improved after discontinuation of use. When starting treatment, protein should be completely avoided.

After regaining consciousness, gradually resume eating small amounts of plant protein, such as soy products. The food is mainly composed of carbohydrates, such as rice porridge, noodle soup, lotus root powder, almond cream, etc., as well as vitamin-rich fruit juice, vegetable juice, etc. For patients who cannot eat orally, they should be given nasogastric feeding or intravenous infusion to supplement high-calorie nutrition and glucose under the guidance of medical staff.

3. Intravenous infusion of branched-chain amino acid solution to correct the imbalance of amino acid metabolism and achieve the purpose of treating hepatic coma.

4. Remove or correct the causes of the disease: such as treating gastrointestinal bleeding and infection, avoiding large amounts of abdominal fluid drainage at one time, avoiding the use of drugs that are toxic to the liver, etc.

5. Nursing of patients with hepatic coma is very important, including preventing bedsores, cleaning the oral cavity, keeping the airway open, and preventing falls.

6. Rational use of drugs: The first principle in drug treatment is to use as little drugs as possible, especially not to use them indiscriminately. The medication must be used under the guidance of a doctor. Neomycin combined with lactulose can prevent hepatic coma. Neomycin works by inhibiting the growth of harmful intestinal bacteria and reducing the absorption of toxins in the intestine. Lactulose makes the intestinal environment acidic, thereby reducing the formation and absorption of ammonia. It can be taken orally or through nasogastric tube. Metronidazole can also be used instead of neomycin for treatment.
