TCM Diagnosis Methods for Ovarian Tumors

The main symptom of this disease is abdominal mass, which is a mass in the abdomen. The first thing to distinguish is the deficiency, excess, symptoms and root cause. New diseases are mainly superficial and excess, with qi stagnation, blood stasis, and phlegm and dampness. Chronic diseases are mainly deficiency of the body, with both qi and yin being damaged. More attention should be paid to the inspection of the abdomen, the size of the abdomen, and the presence or absence of abdominal wall veins to distinguish the deficiency and excess symptoms. The size of the mass, whether it can be moved, and the hardness of the mass can be palpated. In clinical practice, it should also be distinguished from inflammation, tuberculosis-induced masses, and ascites caused by heart and liver diseases.

1. Main symptoms of Qi and blood stasis: lower abdominal mass, hard and fixed, refusing to be pressed, painful when pressed. Irregular vaginal bleeding or amenorrhea, dull complexion, emaciation and ascites, skin and nails are rough, fatigue, dry mouth and no desire to drink, poor urination and defecation, petechiae and ecchymosis on the tongue, and thready and astringent pulse. Analysis: Qi stagnation and blood stasis, if the evil is the cause, the lower abdominal mass is hard and fixed, refusing to be pressed, and painful when pressed; if the blood stasis is blocked by the Chong and Ren meridians, vaginal bleeding or amenorrhea will occur; if the stasis lasts for a long time, new blood will not be generated, and the skin will lose its glory, with a dull complexion, emaciation, and rough skin; if the disease lasts for a long time, the spleen will be deficient and dysfunctional, and there will be ascites, fatigue, and poor urination and defecation. Petechiae and ecchymosis on the tongue, and thready and astringent pulse are signs of Qi and blood stasis.

2. Main symptoms of dampness and toxicity: Rapid enlargement of abdominal mass, abdominal distension and pain or ascites, irregular vaginal bleeding, dry stool, dry and bitter mouth and no desire to drink, dark tongue, thick and greasy coating, stringy or slippery pulse. Analysis: Phlegm and dampness condense, dampness and toxicity accumulate for a long time and turn into heat, which blocks the Qi and causes abdominal mass, abdominal distension and pain or ascites; obstruction of blood vessels, stasis that does not dissipate, damage to the uterus and veins and causes irregular vaginal bleeding; dampness and heat accumulate inside, and the visceral Qi is blocked, resulting in dry stool, dry mouth and no desire to drink. Dark tongue, thick and greasy coating, stringy and rapid pulse are signs of dampness and toxicity becoming severe.

3. Main symptoms of Qi and Yin deficiency: prolonged accumulation of abdominal masses, mental fatigue, emaciation, shortness of breath, laziness, loss of appetite, occasional low fever or a large abdomen like a drum, dilated veins or vaginal bleeding, red or pale tongue, stringy and thin pulse. Analysis: Long-term accumulation of masses, strong evil and weak body, or excessive attack and loss of body fluid and Qi will lead to mental fatigue, emaciation, shortness of breath, laziness, loss of appetite, occasional low fever; Yang cannot transform Qi, and the abdomen is as large as a drum; deficiency with excess will lead to dilated veins, and vaginal bleeding will occur if blood does not return to the meridians. Red or pale tongue and stringy and thin pulse are both signs of Qi and Yin deficiency.
