(II) Determine myometrial invasion during surgery: For stage I cancer with a uterus smaller than normal, or if the patient has limited surgery time due to some reasons, the uterine adnexectomy can be performed first, and the uterine specimen can be dissected to determine whether there is myometrial invasion. Of course, if the specimen is difficult to judge sometimes, subtle changes can be noted under the microscope. (III) Those who are not prepared for lymph node dissection: routinely explore the pelvic and abdominal para-aortic lymph nodes. Those with enlargement should at least undergo a biopsy. If the technical conditions are met and the patient allows, surgical lymph node dissection for endometrial cancer can be performed. The treatment methods used for different diseases are different. The specific contents are introduced in detail below. 1. Treatment of endometrial cancer: It is mostly used for cases of recurrence or metastasis after surgery or radiotherapy. It is also used for patients with well-differentiated adenocarcinoma, early stage, young patients, and those who need to retain fertility. Progestogens are recommended as a component of comprehensive treatment. They can also reduce the postoperative vaginal recurrence rate, so they can also be widely used as adjuvant treatment after surgery or radiotherapy. 2. Surgical treatment of endometrial cancer: Surgery can clarify the scope of the lesion, correctly perform clinical staging, and correctly determine the scope of surgery. For stage I, extrafascial total hysterectomy plus bilateral adnexectomy is usually performed; for stage II, extensive hysterectomy plus bilateral pelvic lymph node dissection is performed. For stage III and IV, surgery is performed first if surgery is possible, and the lesion is removed as much as possible to reduce the tumor. Radiotherapy or treatment is performed after surgery. 3. Radiotherapy for endometrial cancer: For elderly patients or patients with severe medical diseases who cannot receive surgical treatment or are contraindicated for surgery, radiotherapy is still a treatment with a certain degree of efficacy. Radiotherapy includes intracavitary and extracorporeal irradiation. 4. Anti-androgenic drugs for endometrial cancer: Tamoxifen is a non-steroidal anti-androgenic drug that has a slight androgenic effect. It competes with estradiol for receptors (ER), occupies the receptors and plays an anti-androgenic role. After taking this drug, the PR in the tumor increases, which is beneficial to treatment. It is usually used for advanced cases, postoperative recurrence or metastasis. It can be used alone (ineffective treatment) or in combination with chemotherapy drugs. 5. Radiotherapy plus surgery for endometrial cancer: radiotherapy and surgery combined treatment.
