Intramucosal cancer is manifested by the destruction and disappearance of gastric mucosal areas and grooves in the cancerous area, replaced by granular protrusions of varying sizes, mucosal folds concentrated in the cancerous area, the tips of the folds sharply tapered or interrupted, the borders of the cancerous depressions are unclear, and peristaltic waves can pass through. In addition to the above-mentioned changes in gastric areas and grooves, submucosal cancer can be seen with stick-shaped, nodular or bridge-shaped protrusions at the tips of the mucosal folds concentrated in the cancerous area, the edges of the cancerous depressions are clear, and peristaltic waves can still pass through. The current fiber gastroscopy has almost no blind spots, and there is no difficulty in diagnosing advanced cancer. When diagnosing early cancer, it should be noted that early depressed cancer needs to be differentiated from healing benign ulcers; in early flat-surface cancer, the mucosa only changes in color, and the gastric pits and grooves disappear, which is very easy to miss. If necessary, indigo carmine can be spread for contrast staining to show early cancer foci.
The purpose of biopsy is to determine the presence of cancer from a pathological histological perspective. It is usually performed at the same time as an endoscopic examination. Accurate and appropriate sampling is the key to improving the diagnosis rate. The specimen of the ulcer type is taken from the inner side of the ulcer edge wall, and necrotic tissue should not be taken; the specimen of the protrusion type is taken from the outer side of the protrusion, and the concave type is taken from the center. In cases where it is difficult to take a biopsy, cells can be collected by rubbing the lesion with a cell diagnostic brush under the direct vision of a fiber gastroscope.
