For example, granulation tissue hemangioma is a large type of tumor. It is mainly located inside blood vessels and is very difficult to treat. Granulomatous capillary hemangioma, also known as telangiectatic granuloma, capillary hemangioma with infection and lobular capillary hemangioma, is a special type of hemangioma. It is a vascular proliferative lesion that mainly occurs in the mucosa or skin and is characterized by the proliferation of capillaries and the formation of lobule-like structures. Most of them are single polyp-like or nodular lesions with pedicles. They grow rapidly, are brittle, and often ulcerate and bleed. The bottom of the pedicle is connected to the skin or mucosa, forming a collar-like change with normal tissues. It may be misdiagnosed as a malignant tumor clinically.

The cause of granulomatous capillary hemangioma is still unclear. There are currently three main theories: Trauma theory: About 1/3 of cases have been confirmed to be related to trauma. Infection theory: The infection is mainly caused by Staphylococcus aureus, and some people also believe that it is caused by Streptococcus. Hormone expression theory: Estrogen expression is increased in patients with granulomatous hemangioma, and estrogen receptors are expressed in some capillary hemangioma tissues. Some scholars divide the microscopic findings into four types: capillary type, cavernous type, vasodilated type and mixed type.

The cause of granulomatous hemangioma is unknown, and it is generally believed to be related to trauma, infection, and hormone levels. It is more closely related to trauma, and is mostly caused by damage to new blood vessels formed after skin penetrating injuries. It often occurs on the head, face, hands, feet, etc. The diagnosis is not difficult based on the diagnostic characteristics.

Its clinical manifestations are mainly:

1. It grows exophytically, is purple-red, brittle, and easily bleeds when touched. Although the damaged area is not large, the bleeding is difficult to stop.

2. Frequent and repeated bleeding makes ulcers prone to occur on the surface. After each rupture, the tumor will become larger than before, causing a pedicle at the bottom to connect with the skin or mucous membrane, or form a collar-like change with normal tissue.

3. No obvious itching or pain.

Current treatments for granulomatous hemangioma include local sclerotherapy injection, laser therapy, surgical resection, radioactive isotope application, cryotherapy, etc. Surgery is not entirely applicable to lesions on the face and fingers (toes). Excision of facial lesions is likely to leave scars that affect beauty. The incisions after excision of lesions on the fingers and toes are sometimes difficult to suture, and there is also a certain recurrence rate after surgery. Laser therapy is more effective for smaller granulomas, but there is a possibility of bleeding during treatment. The efficacy of radioisotope dressing and cryotherapy is uncertain and they are more likely to leave scars or pigmentation.

Local sclerotherapy injection therapy has been gradually widely adopted because it can be performed in outpatient clinics due to its small wound area, stable efficacy, low scarring and reasonable cost.
