Histopathology Raised scars include hypertrophic scars (hypertrophic scars) and keloids. Their pathological changes are the same except that the range of hypertrophic scars does not exceed the original lesion: in the early stage, fibroblasts in the dermis proliferate, the fibroblasts are active, the fibers proliferate and are arranged in a staggered manner in a whorl or nodular shape, blood vessels proliferate, and there are a small number of inflammatory cells; in the late stage, the collagen fibers in the dermis undergo hyaline degeneration, the collagen bundles become denser and arranged in concentric circles, and the skin appendages atrophy. The tissue pathological changes of hypertrophic scars are different in different development stages, so the treatment plans for raised scars in the early and late stages should be different! The focus in the early stage is to inhibit the proliferation of fibroblasts and blood vessels; the focus in the late stage is to promote the degradation and absorption of collagen fibers. 3. Formation Mechanism The mechanism of raised scar formation It is mainly caused by trauma, after which foreign bodies in the body are not cleaned out and form hyperplasia; after surgical suture, the affected area becomes infected, etc. 4. Classification of causes Raised scars are classified according to their morphology, including hypertrophic scars (hypertrophic scars) and keloids. Raised scars are mostly secondary to skin injuries, such as trauma, infection (acne, herpes zoster, syphilitic ulcers, lupus vulgaris), surgical sutures, burns, injections, vaccinations (vaccination and BCG vaccination), scratching, and even mosquito bites, which can be direct causes. Keloids can also occur spontaneously. 5. Features Raised scars are usually determined by congenital physical conditions. They are the most difficult to treat and are prone to recurrence. Most of them are proliferative lesions of fibroblasts in the dermis of the skin after burns, trauma, surgery or repeated infections. It often occurs after deep burn wounds heal. Reticular proliferative scars are also common at the sutures around the skin graft after skin grafting on third-degree burn wounds. In addition, the most common is the incision scar after any incision is sutured. Hypertrophic scars are characterized by protruding surface, irregular shape, unevenness, redness and congestion, and a firm texture. There is burning and itching sensation.
