How to treat psoriasis

Psoriatic nails are a type of psoriasis, and their symptoms are similar to those of onychomycosis. Psoriatic nail disease causes considerable damage to the nails, which are manifested by the nails losing their luster, turning white, and becoming thicker. The nails of some patients may even break and peel off. After suffering from psoriasis, patients are most concerned about treatment. So, how to treat psoriasis? Let’s take a look at the method below.

Symptoms and signs

Nail lesions are common in psoriasis, with an incidence of 10% to 50%. Common manifestations are as follows:

1. Nail pits are the most common and can affect one to all nails, with fingernails being more susceptible. The depressions are small and shallow, usually not exceeding 1 mm, and are scattered irregularly, occasionally arranged in lines with equal spacing. It is caused by punctate psoriatic changes in the nail matrix that forms the superficial nail plate, with typical pitting due to shedding of the parakeratotic psoriatic stratum corneum as the proximal nail fold doubles.

2. Onycholysis starts from the distal nail edge, but does not exceed 1/2 of the nail. The separated nail plate is grayish yellow. This is due to the large accumulation of blood glycoprotein, which is common when the subungual cuticle and nail bed are inflamed.

3. Subungual thickening: Psoriatic damage occurs in the subungual cuticle and distal nail bed, leading to excessive subungual keratinization.

4. The deck loses its luster, turns white, thickens, becomes uneven, and even breaks and peels off.

5. Splinter hemorrhages are also common in psoriasis. The incidence of nail lesions in non-common psoriasis is higher and the changes are more severe, especially the continuous acrodermatitis in pustular psoriasis.

Medication

Mainly treat skin lesions. When the skin lesions improve, the nail lesions also improve. If nail lesions are the main problem, the following treatment methods can be used:

1.1% fluorouracil solution is used topically to treat hypertrophic and pitted nails twice a day, about 25 ml per month for a total of 6 months.

2. Fluocinolone acetonide night pack, but it is not suitable for long-term use because it will cause atrophy of the periungual soft tissue.

3. Use a needle-free syringe to inject a low concentration of triamcinolone acetonide solution (10 mg/ml) around the nail. The disadvantage is that needle-free syringes are difficult to disinfect and can easily cause viral infections, so it is best not to use them.

4.40% urea ointment bandage treatment: Protect the normal skin around the nail with adhesive tape first, then apply 40% urea ointment (urea 40.0, anhydrous lanolin 20.0, white wax 5.0, white vaseline 35.0) on the diseased nail, cover with plastic paper, seal with adhesive tape, or it is best to use a cut rubber finger glove, and then fix with adhesive tape for 4 to 10 days (average 7.2 days). The diseased nail can become soft. Use simple instruments to separate and remove it.
