2. If the spontaneous toothache occurs more than a week ago, take an X-ray first to see if there is residual pulp in the pulp cavity. If so, perform root canal treatment. If the pain only occurs when biting, check whether the tooth root is fractured. If it is fractured, the tooth cannot be retained and must be extracted.
