Symptoms of poisoning may include persistent mental confusion, severe drowsiness, tremors, slurred speech, staggering, bradycardia, rapid or difficult breathing, severe fatigue, etc. Rescue measures 1. Symptomatic treatment should be carried out as soon as possible, including inducing vomiting or gastric lavage, and maintaining breathing and circulation. 2. If abnormal excitement occurs, barbiturates should not be continued to be used to avoid aggravation of central excitement or prolonged depression of the central nervous system. 3. The use of benzodiazepine receptor antagonist flumazenil can relieve poisoning symptoms. The initial dose of flumazenil for intravenous injection is 0.3 mg. 4. If the required level of awakening is not reached within 60 seconds, the drug may be injected repeatedly until the patient is awake or the total dose reaches 2 mg. Barbiturates Barbiturates also belong to the category of sedatives and hypnotics. Commonly used drugs include phenobarbital, pentobarbital, and theobarbital. This type of drug can cause poisoning (intoxication) when taken orally at a dose of 5-10 times the hypnotic dose, and can cause severe poisoning at 10-15 times the dose. It is life-threatening when the blood concentration is higher than 80-100 μg/ml. Symptoms of acute poisoning are drowsiness, followed by shallow breathing, greatly decreased ventilation, and finally death from respiratory failure. Rescue measures 1. For patients who have taken the drug orally for less than 3 hours, gastric lavage can be performed with a large amount of warm saline or 1:2000 potassium permanganate solution. After washing, use 10-15 g of sodium sulfate as a cathartic. 2. Sodium bicarbonate or sodium lactate can be used to alkalize urine to reduce the reabsorption of this type of drug in the renal tubules. Diuretics such as mannitol can also be used to increase urine volume and promote drug excretion. 3. Respiratory acidosis caused by ventilation dysfunction can promote the drug to pass through the blood-cerebrospinal fluid barrier and aggravate the poisoning reaction. Therefore, it is particularly important to ensure that the respiratory tract is unobstructed. If necessary, tracheotomy or endotracheal intubation is required, oxygen or artificial respiration is given, and central nervous system stimulants may also be administered appropriately. 4. When blood pressure is low, glucose and sodium chloride injection or low molecular weight dextran can be given intravenously. |
