Sex hormone-binding globulin (SHBG), also known as testosterone-estradiol binding globulin, is a carrier of sex hormones. It changes and has significance during the action of sex hormones and in various physiological and pathological conditions. Of the bound testosterone, 44%-60% is bound to SHBG; about 95% of estrogen in the blood circulation is bound to specific SHBG.

Clinical Application:

1. In female hirsutism and virilization, the SHBG content is only 50% of the normal value, while the free T content increases by almost 90%. Therefore, in this type of disease, SHBG determination can not only be used as a diagnostic indicator, but also as a basis for measuring the treatment effect.

2. SHBG levels decrease in polycystic ovary syndrome, obesity, and hypothyroidism.

3. In men with hypogonadism, SHBG levels are elevated, while plasma T levels are often normal. In hyperthyroidism, SHBG levels increase.

4. Liver disease: SHBG levels are elevated in cirrhosis, chronic hepatitis, and fatty liver.

5. The serum sex hormone binding globulin level in patients with premature breast development is elevated. Experimental results show that the increase in SHBG in women with premature breast development is due to low bioactive testosterone, which changes the estrogen/androgen ratio in breast tissue.

Precocious puberty refers to the onset of any sexual characteristic 2 standard deviations earlier than the average age of the normal population. It is generally believed that girls who develop secondary sexual characteristics before the age of 8 or start menstruating before the age of 10, or boys who begin sexual development before the age of 10, can be diagnosed with precocious puberty.
