Disharmony between the liver and stomach and noisy and irritable: The liver is depressed and uncomfortable, and it is caused by the liver wood shaking the middle earth. Therefore, the middle earth is disturbed and restless, and the noise is like hunger. Every time it seeks food to save itself, if it eats a little, the noise will stop a little, but it will come back again. 【Diagnosis, Treatment and Prescription】

(I) Stomach heat and fire

l. Clinical manifestations: noisy and nauseous with acid regurgitation, thirst and preference for cold, bad breath and irritability, or chest tightness and excessive phlegm, and frequent hunger. Or you may feel hungry but not really hungry, with chest tightness and abdominal distension, and no appetite. The tongue is red, the tongue coating is yellow and dry, and the pulse is slippery and rapid.

2. Causes and pathogenesis: Eating too much food can damage the stomach, causing phlegm and dampness to remain inside the body, which can generate heat over time; or liver qi can become blocked, causing qi stagnation and depression, which can turn into fire.

3. Treatment method: Clear the stomach and release heat, harmonize the stomach and resolve phlegm.

4. Prescription: Wen Dan Tang (Qian Jin Yao Fang) with modifications.

Medicines: 10g of ginger and pinellia, 5g of tangerine peel, 10g of Poria, 5g of Citrus aurantium, 10g of Rhizoma Anemarrhenae, 3 slices of ginger, 10g of Coptis chinensis, 10g of fried Gardenia with ginger, and 5g of Licorice.

Meaning of the prescription: Coptis chinensis, Gardenia jasminoides and Rhizoma Anemarrhenae can clear the stomach, relieve heat and reduce counterflow, while Pinellia ternata, dried tangerine peel, Poria cocos, Licorice and Citrus aurantium can harmonize the stomach, resolve phlegm and regulate the middle. Pinellia and ginger are pungent and unblocking, while Coptis chinensis and Gardenia jasminoides are bitter and descending, which have the effect of regulating cold and heat, and harmonizing the rise and fall of qi.

Add or subtract: For those with epigastric fullness and chest tightness, add Scutellaria baicalensis to relieve heat; for those with acid regurgitation, add Evodia rutaecarpa and White Peony Root to stop the acid; for those with food stagnation, add Chicken Gizzard Stone, Crataegus pinnatifida, and Shenqu to help digest food and relieve stagnation.

5. Alternative method: If there is noisy feeling, vomiting, fullness in the abdomen, spitting out clear saliva, pale tongue with white coating, it is due to phlegm and cold. You can use Fuling Drink (Wai Tai Mi Yao), which contains Fuling, Atractylodes macrocephala, Ji Shi, tangerine peel, ginger, and ginseng. The above prescription is used for stomach heat and phlegm, while this prescription is effective for stomach cold.

(ii) Weak spleen and stomach

1. Clinical manifestations: Noisy feeling comes and goes, lessens after eating and comes back after eating, tasteless mouth, occasional drooling, fatigue and weakness. The tongue is pale red and the pulse is weak and slow.

2. Causes and pathogenesis: Insufficient spleen and stomach, poor Qi movement, disturbance of the central palace, and indigestion of food.

3. Treatment method: tonify Qi and strengthen the spleen, harmonize the stomach and regulate the middle.

4. Prescription: Liujunzi Decoction (Medical Biography) with alkali added.

Medicines: Codonopsis pilosula 10g, Atractylodes macrocephala 10g, Poria 10g, Pinellia ternata 10g, Tangerine peel 5-10g. 5g of roasted licorice root.

Meaning of the prescription: Codonopsis pilosula, Atractylodes macrocephala, Poria cocos, and licorice strengthen the spleen and replenish qi, while Pinellia ternata and dried tangerine peel harmonize the stomach and reduce reflux.

Add or subtract: For those who cannot taste the food, add cardamom seeds and magnolia flowers to regulate qi and stimulate appetite; for those who vomit and drool, add Evodia rutaecarpa and ginger to stop vomiting; for those with abdominal distension, add costus root and amomum villosum to regulate qi.

5. Alternative method: For those who suffer from stomachache when hungry, use Huangqi Jianzhong Decoction (from Golden Chamber Synopsis) with modifications to strengthen the middle part of the body, replenish deficiency, relieve acute pain and stop pain. If the patient has spleen deficiency, noisy stools and loose stools, use Liushen Powder (Sanyin Fang), which is Sijunzi Decoction with yam and lentils to strengthen the spleen and nourish the spleen yin.

(III) Insufficient stomach yin

1. Clinical manifestations: intermittent noise, dry mouth and tongue. No appetite or not eating when hungry, feeling bloated after eating. Very convenient. The tongue is red, with little or no coating, and the pulse is thready and rapid.

2. Etiology and pathogenesis: insufficient stomach yin and loss of body fluid. Dryness and heat are endogenous.

3. Treatment method: Nourish yin and harmonize the stomach, promote fluid production and moisten dryness.

4. Prescription: Yiwei Decoction (Treatise on Febrile Diseases) with modifications.

Medicines: Adenophora 10-15g, Ophiopogon japonicus 10-15g, Rehmannia glutinosa 10g, Polygonatum odoratum 10g, Paeonia lactiflora 10-15g. 5g licorice.

Meaning of the prescription: Adenophora Root and Ophiopogon Japonicus nourish yin and harmonize the stomach, Radix Rehmanniae Root and Polygonatum Odorifera produce body fluid and relieve dryness, while Paeonia lactiflora and Glycyrrhizic Acid sweeten yin.

Add or subtract: For those with dry mouth and tongue, add Dendrobium and Asparagus cochinchinensis; for those with poor appetite, add Prunus mume and Atractylodes lancea; for those with dry stool, add Torreya grandis seed.

5. Workaround: You can also use Maimendong Decoction (Golden Chamber Synopsis) with modifications. The above prescription focuses on nourishing yin and producing body fluid, while this prescription focuses on harmonizing the stomach and relieving counterflow.

(IV) Liver depression and blood deficiency

1. Clinical manifestations: noisy and distended abdomen, feeling hungry but not really hungry, chest tightness and flank distension. Depression or irritability, sometimes acid reflux or vomiting, or bitter and dry mouth. The tongue coating is thin and the pulse is stringy.

2. Causes and pathogenesis: disharmony of seven emotions, stagnation of liver qi, and disharmony of stomach qi.

3. Treatment method: Soothe the liver and regulate qi, strengthen the spleen and calm the wind.

4. Prescription: Xiaoyao Powder (Ju Fang) combined with Zuojin Pills (Dan Xi's Heart Method).

Medicine: Bupleurum 10g, White Peony 10g, Angelica 10g. Poria 10g, Atractylodes 10g, 3 slices of ginger, 3g of mint, and 3-5g of licorice. 3-5g of Coptis chinensis and 2-3g of Evodia rutaecarpa.

Meaning of the prescription: Bupleurum soothes the liver and regulates qi, Angelica and white peony root harmonize blood and nourish the liver, Poria and Atractylodes macrocephala strengthen the spleen and eliminate dampness, peppermint and ginger harmonize the ascending and descending, Coptis chinensis and Evodia rutaecarpa relieve the liver and harmonize the stomach.

Add or subtract: Bitter taste in the mouth and irritability, add fried Gardenia to relieve heat if there is fire; chest tightness and flank distension. For patients with qi stagnation, add Xiangfu to regulate qi; for patients with greasy tongue coating, poor appetite and phlegm and dampness, add Chenpi and Atractylodes to resolve phlegm; for patients with food stagnation, add Shenqu and malt to help digestion. That is Heyuejiwan (Danxi's Heart Method).

5. Alternative method: If the patient has weak blood and qi, insufficiency of the heart and spleen, dull complexion, palpitations, insomnia, dizziness, forgetfulness, pale tongue and weak pulse, Guipi Decoction (Ji Sheng Fang) can be used with modifications. For those who suffer from both liver depression and heart and spleen deficiency, Xiaoyao Powder combined with Guipi Decoction can be used.
