There are many common skin diseases, such as viral skin diseases (such as chickenpox), bacterial skin diseases (such as folliculitis), fungal skin diseases (tinea pedis and tinea manuum), skin tumors (leukoplakia), etc., about 16 types. Severe skin ulcers are more common, and the main manifestations are skin wounds that cannot heal, defects in the skin and surface tissues, liquefaction, infection and necrosis. Since this disease will appear on the surface of the skin and cause sensory discomfort, the patient is under tremendous psychological pressure. In addition, this disease is a chronic disease, which has a great impact on the patient's life. Many patients with skin ulcers do not know how to clean the affected areas. Some patients cannot be admitted to the hospital for certain reasons and have to undergo treatment and care at home on their own. Therefore, this disease is extremely torturous both mentally and physically. So, how should it be treated and cared for? 1. Local treatment ① Debridement of ulcers; removal of necrotic tissue, scars, old granulation tissue, foreign matter, etc., until relatively healthy, well-vascularized tissue is left. For patients with chronic ulcers with exposed bones and joints who need flap repair, the quality of wound treatment is one of the keys to the success of flap transplantation. Soak and rinse with hydrogen peroxide, saline, and iodine for 5 minutes respectively. Cover with iodine gauze. If local edema is severe, apply 3% hypertonic saline wet compress, or add antibiotics and antifungal drugs based on the results of bacterial and fungal culture and examination. ② Treatment of skin eczema: Fully follow the principles of using topical dermatological drugs: In the acute phase, when there is more exudation, apply 3% boric acid solution for wet compress; when there is less exudation, use glucocorticoid cream; in the chronic phase, use halometasone cream or Bactroban ointment. ③ Local therapy: local irradiation such as helium-neon laser, ultrashort wave, infrared, etc. ④Surgical treatment: For patients with large ulcer area, long-term unhealed ulcers, or those with exposed bones, joints, tendons or steel plates, surgical treatment may be considered. Systemic treatment 2. Systemic treatment The purpose is to fight inflammation, relieve itching and improve the body's resistance. ① Application of antibacterial or antifungal drugs: based on the results of bacterial and fungal culture and examination, intravenous antibiotics and antifungal drugs are used; ② Use antihistamines, calcium, vitamin C, etc.; ③Medical substances that enhance the body's resistance: such as Utilins, etc.; ④Correct anemia or hypoproteinemia. 3. Targeted fluid therapy For the treatment of skin ulcers, transdermal administration of drugs directly applies the drugs to the affected area. The drugs can penetrate from the outside to the inside without damaging the liquefied and eliminated necrotic tissue, making it unfavorable for bacterial colonization and reproduction. Moreover, only the purulent secretions of the bones and soft tissues are drawn out of the body from the diseased area or wound cavity using the body's body fluid as a carrier and converted into physiological body fluid, thereby improving the acidic environment of the diseased area. Since inflammation damages the surrounding normal blood vessels and soft tissues and affects the normal blood supply, the active molecules contained in the medicine can balance the inflammatory response and enable the affected area to continuously excrete pathological body fluids and continuously renew them into healthy physiological body fluids, thereby ensuring the patency of blood vessels and lymphatic vessels and a good blood supply. |
