Guidelines for antiviral treatment of hepatitis B

Antiviral treatment is recommended for hepatitis B patients with HBV-DNA(+) and/or HBeAg(+) and abnormal liver function. The purpose is to inhibit the activity and replication of HBv and reduce or inhibit damage to liver cells, not to eliminate the virus. It should be pointed out that current antiviral treatment is ineffective for hepatitis B virus carriers (i.e., those with normal liver function tests). The incidence of cirrhosis and liver cancer in this group of people is extremely low, so antiviral treatment is not recommended. However, liver function status must be monitored regularly to detect hepatitis activity in a timely manner. Currently proven effective antiviral drugs include:

①α-interferon (IFNα).

②Nucleoside analogs such as lamivudine have been widely used in clinical practice, and some nucleoside analogs such as famciclovir, adefovir, and stecavir are also being used in clinical practice.

③ Oxymatrine is a bioalkaloid extracted from the plant Sophora flavescens. It has anti-HBV effects and has mild adverse reactions. Currently, there are two dosage forms: injection and oral solution.

Combination therapy for antiviral treatment is an inevitable trend in the future, and the following combinations are possible:

① The combined use of IFNα and glycoside analogs. It is currently believed that the combined treatment of the two has a higher HBeAg seroconversion rate than single drug use and can reduce HBv mutation. The effect will be better if traditional Chinese medicine for liver protection is added, such as Compound Ant Liver Nourishing Capsules.

②Combined use of various nucleoside analogs.

③ Combined use of IFNα or nucleoside analogs and immunopotentiators.

④Domestic antiviral drugs and the above combined use.
