Alcohol: According to liver puncture biopsy of long-term alcoholics, 75%-95% have fatty infiltration. Studies have shown that there is a dose-response relationship between alcohol consumption and the incidence of fatty liver. Drinking more than 80-160 grams of alcohol per day will increase the incidence of ethanolic fatty liver by 5-25 times. Its pathogenesis is currently believed to be the result of multiple factors. First, a large amount of ethanol in the body can inhibit the synthesis of mitochondrial proteins, reduce the ability of mitochondria to oxidize fat, cause accumulation of fatty acids in liver tissue, accumulation of ketone bodies in the body, and increase the ratio of lactate to pyruvate in the body. Excessive lactate inhibits the excretion of uric acid by the kidneys, causing hyperuricemia, reducing liver glycogen heterogeneity, leading to hypoglycemia, and some patients suffer sudden death. In addition, ethanol metabolites such as acetaldehyde also have direct toxic effects on liver tissue itself, causing fatty degeneration of liver tissue. 3. Insufficient protein and calories: Malnutrition and lack of protein, such as gastroenteritis, are important causes of fatty liver. It is often seen in cases of insufficient food intake or digestive disorders, which result in the inability to synthesize apolipoproteins, leading to the accumulation of triglycerides in the liver and the formation of fatty liver. When starving, serum free fatty acids increase, which can lead to the formation of fatty liver. The mechanism may be related to lack of glucose acquisition, increased growth hormone and increased sympathetic nerve excitability, and the degree of hepatic fatty degeneration depends on the length of starvation. After jejunal bypass surgery and gastric segmentation, increased fat mobilization in the body can also lead to fatty liver. 4. Diabetes: An average of 50% of diabetic patients may develop fatty liver, especially those who develop diabetes between the ages of 40 and 50 are more likely to have fat deposition in the liver. Its pathogenesis is not very clear at present, and it may be related to the increased plasma insulin levels and increased concentrations of unesterified fatty acids in the patient's plasma. |
