What is the cause of the red and itchy coccyx groove

Causes

1. Mental factors

3. Local stimulation

Friction caused by a hard collar, stimulation from chemical substances, insect bites, sunlight, scratching, etc. can all induce the occurrence of this disease.

Clinical manifestations

1. When the disease first occurs

There is only an itchy sensation, but no primary skin lesions. Due to scratching and friction, flat papules ranging in size from millet to mung bean gradually appear on the skin. They are round or polygonal, hard and shiny, light red or normal skin color, and scattered. Due to paroxysmal severe itching, patients often scratch, and the papules gradually increase in number. Over time, they merge into flakes, become thickened and lichenified, and are manifested by deepening of skin wrinkles and raised skin ridges. The skin lesions become dark brown, dry, and have fine desquamation. The patchy lesions have clear borders, and there may be small flat papules at the edges, which are scattered and isolated. The number of skin lesions is variable and they may be solitary or widespread, of varying sizes and shapes.

2. Neurodermatitis

It often occurs on both sides of the neck, nape, elbows, popliteal fossa, sacrum, wrists, ankles, and also on the lower back, eyelids, limbs and vulva. If the skin lesions are limited to one or a few places, it is called localized neurodermatitis; if the skin lesions are widely distributed, even throughout the body, it is called generalized neurodermatitis.

3. Subjective symptoms

It is a paroxysmal severe itching, especially at night, which affects sleep. Scratching may leave blood marks and blood scabs, and in severe cases, folliculitis and lymphadenitis may occur.

4. This disease is a chronic disease with symptoms that vary in severity and are prone to relapse after recovery.

Classification

1. Localized neurodermatitis

More than 90% of cases occur on the neck, followed by the elbows, sacrum, eyelids, popliteal fossa, etc. The patient first feels local itching, followed by the appearance of clustered millet- to rice-sized, normal skin-colored or light brown or light red polygonal flat papules, which are slightly shiny and covered with a small amount of chaff-like scales. The papules then merge into pieces. Due to the itching, the patient often scratches, which stimulates the skin to gradually thicken and form lichenification with clear boundaries. Scratches are common around the affected skin lesions, and blood scabs are formed.

2. Disseminated neurodermatitis

The skin lesions are similar to those of localized neurodermatitis, but are widely distributed, affecting the head, limbs, trunk, etc., with paroxysmal severe itching, especially at night, affecting sleep. The course of the disease is chronic and prone to recurrence. Frequent scratching may lead to eczematous changes or secondary infections such as folliculitis and furunculosis.
