Treatment of hypokalemia in renal tubular acidosis

1. Eliminate factors that induce aggravation, such as obvious diuretics and drugs that are harmful to the kidneys. If you have stones or urinary tract obstruction, you should seek early treatment.

2. Potassium salt supplementation is usually potassium citrate taken orally, usually 20 ml, 3/d. The dosage varies depending on the blood potassium level and needs to be maintained long-term. However, potassium chloride should not be taken orally. Only when obvious hypokalemia causes life-threatening arrhythmias can potassium chloride be dripped intravenously and the dose should be stopped immediately when the blood potassium level reaches 3.5mmol/L.

3. Correct metabolic acidosis and supplement potassium salt at the same time. The dosage of oral or intravenous sodium bicarbonate should be determined based on the blood bicarbonate level, respiratory compensation ability, and blood pH value. For mild cases, 1.0, 3/d should be used. You can also take sodium citrate mixture 10-20 ml orally 3/d for a long time.

4. To correct osteoporosis, you can take vitamin AD pills orally for a long time, α-D 30.5μg, 1/d, and take calcium supplements at the same time. Serum calcium levels need to be monitored regularly to prevent hypercalcemia. Nandrolone phenylpropionate can also be injected intramuscularly to promote bone growth.

5. Traditional Chinese medicine can be used to treat kidney yin deficiency or kidney yang deficiency, such as Liuwei Dihuang Pills, Jinkui Shenqi Pills, Fudihuang Pills, etc.
