Lumbar third transverse process syndrome is a lumbar disease that causes considerable pain to patients. So what causes this disease? What are the things that require attention? Third Lumbar Transverse Process Syndrome is low back pain or lumbosacral pain caused by acute or chronic injury to the tip of the L3 transverse process.

Most patients with symptoms are young and middle-aged, often with a history of lumbar sprain or strain, low back pain or pain in the waist and buttocks, which is aggravated during activities. Some patients may have pain radiating from the ipsilateral erector spinae muscle (spinalis muscle) to the thigh. Or it may be accompanied by pain above the knee, but rarely the pain radiates beyond the knee joint to the calf; a few may experience pain in the medial thigh or lower abdomen, but without tenderness.

2. Physical signs: Spasm of the erector spinae muscles on the affected side and obvious localized tenderness at the tip of the L3 transverse process. In the early stage, the gluteal muscles are plump and the adductor muscles are spasmodic (innervated by the L2-4 obturator nerve). In severe cases, gluteal muscle spasm can be seen in the late stage, and cord-like objects can be felt in the gluteus medius muscle, which is tender.

3. Differentiating points between this disease and lumbar disc herniation: ① In this disease, the pain does not increase when coughing or sneezing. ② The location of the tenderness point is different. This disease is located at the tip of the L3 transverse process, and the latter is the interlaminar space of the affected vertebra. ③ A small number of severe patients with this disease may have a positive leg raising test, but the enhanced test will definitely be negative.

Rest and exercise. Take adequate rest and avoid exerting force on your waist. Adhere to rehabilitation gymnastics exercises every morning and evening, do them moderately and in appropriate amounts, move slowly, and stay for 3-5 seconds. Take good care during exercise and daily life to prevent moisture, cold, fatigue, acute sprains, long periods of sitting or standing, and weight bearing. 2. Inject 2% lidocaine 5-10ml plus prednisolone acetate 25mg/1ml into the painful area using a syringe with a long needle. While injecting the painful point, the injection needle is used to perform soft tissue peeling at the tip of the L3 transverse process, with significant therapeutic effects.

3. Medication: Use anti-inflammatory and analgesic drugs such as ibuprofen, voltaren, etc., and blood-activating and blood-stasis-removing drugs such as Panax notoginseng blood-activating tablets, etc.

4. Physical therapy with excited energy electricity, computer medium frequency (for those with normal heart and blood pressure) microwave, nano wave, computer multi-function fumigation, ultrashort wave electrotherapy machine, ultrasonic therapy, ultra laser, etc.

The onset of this disease is actually predictable, it is caused by some injuries or bad living habits, so we should pay attention to it in our daily life. Also, this disease is difficult to treat, but it is not untreatable.
