If the cardiac index drops below 3L/(min·m2) and there is peripheral vasoconstriction and insufficient tissue perfusion, it is called low cardiac output syndrome. Low cardiac output syndrome is a common and common cardiac pump dysfunction caused by myocardial damage in the early stage after open heart surgery, accompanied by the response of surrounding tissues to the low perfusion state. It is one of the main causes of early death in postoperative patients.

The main causes of his disease are: 1. Unsatisfactory correction of heart malformation is an important cause of postoperative low cardiac output;

2. Insufficient blood volume, or insufficient effective circulating blood volume;

3. During the intracardiac operation, the heart circulation needs to be blocked. Ischemia and hypoxia can damage the myocardium and cause myocardial incomplete contraction;

4. Insufficient oxygen exchange, hypoxia or acidemia after surgery can aggravate myocardial insufficiency;

5. Tachycardia or bradycardia affecting atrioventricular diastole, arrhythmia such as third-degree block caused by hypoxia or surgical trauma are also often the cause of postoperative low discharge;

6. Cardiac compression affects ventricular filling, such as pericardial tamponade or tightness after pericardial suture, which is also one of the reasons for postoperative low discharge;

7. Myocardial infarction caused by coronary artery insufficiency and coronary artery gas embolism are occasional causes;

8. Children with poor heart function before surgery have poor general condition, and their heart, lung, liver and kidney functions are impaired to varying degrees, making them prone to low cardiac output syndrome;

9. Patients with large left-to-right cardiac shunt are prone to pulmonary hypertension. Patients with severe pulmonary hypertension have sclerosis of the walls of pulmonary arterioles, thickening of the walls, and narrowing of the lumens, often accompanied by thickening of the tissue between the alveoli and capillaries, and interstitial edema, which increase pulmonary vascular resistance and right ventricular hypertrophy and enlargement. The balance of myocardial oxygen supply and demand is already in a compensatory state before surgery, and improper intraoperative management may also become a contributing factor to low cardiac output syndrome. It is recommended that congenital heart disease be detected and treated as early as possible.
