Treatment of navicular fracture

I believe some friends have had such an experience, that is, navicular fracture. The mid-tarsal joint composed of the navicular, cuneiform and cuboid bones, also known as the transverse tarsal joint, is easily dislocated due to trauma. Although simple bone fractures mentioned above are not common, they are not rare. It accounts for about 0.3% of all fractures in the body and should still be paid attention to. Now I will introduce to you its treatment method.

Treatment with medication: 1. For those without displacement, fix the calf with plaster for about 6 weeks. For those who have not healed, the period can be extended appropriately. Strengthen functional exercises after removing the plaster.

2. For those who have displacement but can achieve satisfactory alignment, the previous method should be used for treatment after reduction.

3. Patients with severe displacement, including those with failed reduction, require open reduction + internal fixation (Figure 1), supplemented by calf plaster immobilization.

(1) Scaphoid fracture: After reduction, cross fixation with Kirschner wires can be performed.

(2) Scaphoid tuberosity avulsion: For smaller bone fragments, No. 10 thread can be used to suture together with the attachment of the posterior tibialis muscle. For larger bone fragments, small screws or Kirschner wires can be used to fix them.

(3) Avulsion fracture of the dorsal margin of the scaphoid: For patients with difficult fixation after open reduction, the scaphoid can be resected.

4. Old injury The basic principle is similar to that of talus fracture. If accompanied by traumatic arthritis or ischemic necrosis, arthrodesis may be performed as appropriate. During the operation, the talonavicular joint was preserved as much as possible, while the navicular cuneiform joint was fused (Figure 2).

Diet therapy:

(1) Avoid eating too much meat and bones.

(2) Avoid excessive consumption of white sugar.

(3) Avoid indigestible foods. Avoid eating foods that are easy to cause bloating or indigestion, such as sweet potatoes, taro, and glutinous rice. It is better to eat more fruits and vegetables.
