She is 49 years old and a Uyghur herdsman. A month ago, she felt a dull pain in her abdomen and a mass was visible on her abdominal wall. I started seeing a doctor at the health center 3 weeks ago, but there was no improvement. The lump is getting bigger and bigger.

Starting from January 1st, I went to major hospitals for treatment. A lot of tests including X-ray, B-ultrasound, CT, MRI, etc. were done, which only showed that there was a huge cystic mass on the abdominal wall, but there was no clear diagnosis. A doctor once suspected an abscess, but the skin on the patient's abdominal wall was not red or swollen, so it didn't look like an abscess. The doctor who saw me kept doing all kinds of tests I could think of, but was still confused.

She came to see me near the end of get off work today. After briefly asking about her medical history, I asked her to lie on the examination bed and performed a careful physical examination on her. The bulge on her abdominal wall was as big as a fist, and there was a very obvious sense of fluctuation. This means that there is fluid inside the lump. I asked the resident doctor to get a sterile cotton ball and a 20 ml syringe, perform a simple puncture, and draw out 20 ml of pus. The patient's diagnosis was clear: abdominal wall abscess. These pus can be checked for bacteria and drug sensitivity tests can be performed. There will be no problem in the patient's treatment.

Looking at the pile of tests that the patient handed me, I really wanted to knock those doctors on the head: They didn't do the most basic physical examination and puncture, and only relied on the examinations. Is that okay?

The patient is a herdsman living deep in the mountains. It takes her two hours to get to Bozidun Township Government by motorcycle, and another three hours to get to the county seat. So it is easy for her to be delayed in seeing a doctor. The abdominal wall abscess has been delayed for a month, and the redness and swelling on the skin surface has subsided, and of course the typical symptoms of inflammation such as redness, swelling, heat and pain are no longer there.
