Subcutaneous fibrous tissue hyperplasia

1. Introduction to fibrous hyperplasia

Fibrous hyperplasia Tumors and tumor-like hyperplasia of fibrous tissue are common lesions among oral soft tissue tumors.

Fibrous hyperplasia is a common fibrous lesion in the oral cavity and is a reaction to irritation and repeated trauma. The lesions may be pedunculated or sessile, smooth or fissured, and may occur anywhere in the oral cavity. Most lesions are rich in collagen fibers and relatively poor in cells. Except when ulcers and inflammation occur, there are generally not many blood vessels. With age, the number of cells in these lesions decreases, and inflammatory angiogenesis also decreases accordingly. It is most common in the oral cavity, especially in the gums. The proximal fibrous hyperplasia is often located in the interdental papilla. The cause of the disease may be subgingival calculus or irritation from the sharp edges of adjacent caries. The color may be normal or dark red, depending on the number of blood vessels and the degree of inflammation. The surface of the fibrous hyperplasia is covered with mucosa and ulcers may occur.

2. Symptoms of fibrosis

Lesions on the lips and tongue are well-defined nodular masses. Histologically, fibrous hyperplasia is mostly higher than collagen fibers and relatively lacks cells. Vascularization is rare except for ulceration or marked inflammation. As people age, the number of cells in these lesions decreases, and the number of inflammatory blood vessels also decreases accordingly.

Clinically, the most prominent fibrous hyperplasia is located between the teeth, parallel to the teeth, and protrudes outward. Therefore, chewing or biting things can easily irritate the joints and cause bleeding. The discomfort caused by fibrous hyperplasia varies depending on where it occurs.

3. Surgical treatment of fibrous hyperplasia

No matter where fibrosis occurs in the oral cavity, laser can easily remove the hyperplastic lesions. Moreover, there is no bleeding during the operation, and better therapeutic effects can be achieved with relatively small injuries. It is more ideal that the surgical area is clean and the pain is minimal.

Use 1% to 2% lidocaine and a No. 4 or 5 small needle to inject at the base of the hyperplastic focus. Clamp a sterile cotton ball to slightly compress the injection site, then use CO2 laser (power 15W, focused) or Nd:YAglaser (power 15-20W, polished handle, homemade) to directly vaporize or cut at the base. If the cutting power is too high, adjust it lower appropriately to avoid damaging tooth and gum tissue during the operation. If the cause is stimulation from the sharp edge of an adjacent carious tooth, the sharp edge of the carious tooth can be flattened with a laser while vaporizing or cutting the fibrous hyperplasia. Otherwise, recurrence is likely to occur if stimulated again after surgery. Fibrous hyperplasia occurring in the hard and soft palate is directly vaporized by a laser knife or contact-cut by a fiber optic knife head. When CO2 laser is used to cut fibrous hyperplasia deep inside the incision, the operation must be carried out with caution to avoid damaging normal tissues other than the lesion. After surgery, apply gentian violet to the surgical area.
