Surgical treatment of hilar cholangiocarcinoma Currently, surgical resection is still the most effective means of treating extrahepatic cholangiocarcinoma. However, the biological behavior of cholangiocarcinoma determines its clinical characteristics of low resection rate, especially hilar cholangiocarcinoma. Since it is located in the extrahepatic bile duct and special location, it is often in the late stage once diagnosed, so surgical resection is more difficult. 2. Surgical treatment of middle and lower bile duct cancer For bile duct cancer located in the middle segment, if the tumor is relatively localized, the common bile duct where the tumor is located can be partially removed. The key to the surgical effect is whether the hepatoduodenal ligament can be "skeletonized" and cleared. However, some scholars believe that middle and lower bile duct cancer is more malignant, develops rapidly, and is prone to metastasize to the retropancreatic and peri-abdominal lymph nodes. Radical resection should include extensive resection of the gallbladder, common bile duct, pancreatic head and duodenum, plus thorough clearance of the hepatoduodenal ligament. This issue should be treated accordingly based on the principle of "individualization" for different patients, and cannot be generalized. 3. Other treatments Radiotherapy for cholangiocarcinoma: In the past, it was believed that cholangiocarcinoma was not sensitive to radiotherapy, and radiotherapy was even abandoned clinically. In the past 20 years, radiotherapy for cholangiocarcinoma has been widely studied. Some literature reports that radiotherapy can relieve the symptoms of cholangiocarcinoma patients and may prolong survival, and has achieved certain results. Radiotherapy can be used as the main treatment for cholangiocarcinoma, and can also be used as an adjuvant treatment for surgery. The main methods include traditional external irradiation therapy, intraoperative irradiation therapy, internal irradiation therapy and radioimmunotherapy.
