How to prevent endometrial cancer

1. Raise cancer awareness

In daily life, we should carry out and participate in more cancer prevention publicity and surveys, strengthen health and medical knowledge, and educate women with abnormal menopausal bleeding, vaginal discharge, obesity, hypertension or diabetes to be vigilant, seek medical treatment in time, and get early diagnosis. This is the most basic way to prevent endometrial cancer in daily life.

2. Treatment History

If any gynecological disease is found, it should be treated promptly and followed up closely. If the treatment is not effective, the uterus should be removed surgically in time. If the patient already has children, has no hope of having children, or is older, conservative treatment is not necessary. The uterus can be removed directly.

3. Estrogen therapy

Use estrogen in moderation and strictly follow the indications for its use. Menopausal women should use estrogen for replacement therapy under the guidance of a doctor, and at the same time use progesterone to regularly transform the endometrium.

IV. Monitoring of high-risk groups

Patients with endometrial cancer should be followed up closely and regularly after treatment to detect recurrence as early as possible. About 75% to 95% of recurrences occur within 2 to 3 years after surgery. Routine follow-up should include a detailed medical history (including any new symptoms), pelvic examination, vaginal cytology smear, chest X-ray, serum CA125 test, blood routine, blood chemistry test, etc. CT and MRI examinations can be performed if necessary. Generally, follow-up is conducted every 3 months within 2 to 3 years after surgery, every 6 months after 3 years, and once a year after 5 years. 95% of recurrence cases can be detected by clinical examination, vaginal cytology smear and serum CA125 test.

What is the surgical treatment for endometrial cancer?

(I) Ascites or peritoneal washing fluid is used to search for cancer cells. After the peritoneum is cut open, if there is ascites, it is collected and centrifuged to search for cancer cells. If there is no ascites, 200 ml of normal saline is injected into the peritoneal cavity to wash the peritoneal cavity, and the washing fluid is sucked out and centrifuged to search for cancer cells. In addition to surgery, other auxiliary treatments should be given to those who find cancer cells.

(II) Determine myometrial invasion during surgery: For stage I cancer with a uterus smaller than normal, or if the patient has limited surgery time due to some reasons, the uterine adnexectomy can be performed first, and the uterine specimen can be dissected to determine whether there is myometrial invasion. Of course, if the specimen is difficult to judge sometimes, subtle changes can be noted under the microscope.

(III) Those who are not prepared for lymph node dissection: routinely explore the pelvic and abdominal para-aortic lymph nodes. Those with enlargement should at least undergo a biopsy. If the technical conditions are met and the patient allows, surgical lymph node dissection for endometrial cancer can be performed.
