How to care for patients after bladder cancer resection

Bladder cancer is a common urinary system disease. Some patients with severe conditions require total cystectomy, ileal bladder surgery, and permanent placement of a urine collector. During care, attention should be paid to observing urine changes, using urine collectors correctly, prohibiting the use of drugs that promote intestinal motility, protecting the skin around the fistula, and observing abnormal phenomena.

1. Observe the changes in urine: Pay attention to the changes in urine after surgery. If you find flocculent mucus in the urine, drink more water and take baking soda tablets orally to alkalize the urine and thin the mucus, so as to facilitate urination.

2. Correct use of urine collector: The urine collector consists of a base and a urine bag. Generally, the base is replaced every few days, and the urine bag is replaced every 1-2 days. When using a urine collector, patients should pay attention to dumping urine in time and record the urine volume. The urine bag and drainage position should be lower than the pubic symphysis to prevent retrograde infection caused by urine reflux. When leaving bed for activities, the catheter and urine bag should be properly placed.

3. Do not use drugs that restore intestinal motility: After cystectomy, patients can eat after the anus is exhausted. The patient's intestinal motility has not yet recovered, so drugs that promote intestinal motility recovery, such as neostigmine, should be prohibited to prevent damage to the ureteral anastomosis.

4. Protect the skin around the fistula: People with permanent skin fistulas should pay attention to protecting the skin around the fistula, clean and disinfect it every day, and apply zinc oxide ointment under the guidance of a doctor.

5. Observe abnormal phenomena: After cystectomy, you should pay attention to observe abnormalities of the patient. If there is sudden high fever or bloody secretions at the urethral orifice, you should be alert to the possibility of residual or occurring urethral tumors and go to the hospital for treatment in time.
