Compound retinoic acid ointment

If you have suffered from skin diseases, you will probably be familiar with compound retinoic acid ointment. This is mainly a medicine used to treat blackheads and acne, and it also has a good effect on pigmented skin diseases. Since compound retinoic acid ointment is a relatively irritating drug, it may cause adverse reactions after use.

The first is efficacy and indications.

Acne vulgaris, especially blackhead comedones, senile, solar or drug-induced skin atrophy, ichthyosis and various abnormal keratinization and hyperpigmentation skin diseases, psoriasis.

The second is the chemical composition.

Chemical name: 3,7-dimethyl-9-(2,6,6-trimethylcyclohexene)-2,4,6,8-all-trans-nonatetraenoic acid

The third is the pharmacological effect.

It is possible that this is achieved by regulating the mitogenic effect of epidermal growth factor, and the affected epidermal cells can show an increase in DNA synthesis and mitotic index. Retinoic acid can significantly inhibit the formation of experimental acne and remove keratin plugs by regulating the abnormal keratinization process of the sebaceous gland epithelium of the hair follicles, thereby preventing and eliminating acne skin lesions. Retinoic acid can affect the melanin production of melanocytes.

Its action is multi-site, thereby reducing melanin formation and lightening skin pigmentation. Retinoic acid has no effect on the tyrosinase activity and melanin composition of normal human melanocytes. When the skin undergoes physiological aging, or is damaged by drugs, ultraviolet radiation, and trauma, it stimulates the synthesis of skin extracellular matrix proteins, accelerates the formation of new connective tissue bands in the upper dermis, and increases the tension strength of the wound site. Retinoic acid has no effect on normal skin collagen synthesis. In addition, retinoic acid has inhibitory activity on leukocyte chemotaxis, thereby exerting an anti-inflammatory effect.

All-trans retinoic acid has no direct effect on sebaceous glands and their secretion. Toxicology: Oral retinoic acid has strong teratogenic effects on experimental animals (including mice, rats, hamsters, rabbits, monkeys, etc.) and humans. And can cause maternal systemic toxicity. However, retrospective data to date have not found any teratogenic effects following topical application of the drug to human skin. Retinoic acid is irritating to the skin. The skin reactions of the above-mentioned experimental animals are significantly more severe than those of humans, and may cause varying degrees of skin irritation, inflammation, redness, swelling, and erosion depending on the drug concentration and number of doses, weaken the stratum corneum barrier, increase drug absorption, and cause systemic toxicity. Although it is irritating when applied externally to human skin, it does not have the serious reactions mentioned above.

The fourth is contraindications.

It is contraindicated for women in the first 3 months of pregnancy. It is contraindicated for patients with acute or subacute dermatitis, eczema or other skin diseases.
