First, infection with high fever:

We observed 50 patients with black tongue coating, and 36 of them had fever of varying degrees, including 20 with high fever and 16 with low fever. Most people develop black tongue coating during the process of fever. For some people, the black tongue coating disappears after the high fever subsides, but reappears when the fever occurs for the second time, proving that there is a certain relationship between fever and the appearance of black tongue coating. In addition, black tongue coating is also related to the length of time of fever. In the above cases, black tongue coating appeared after 4 days in the shortest fever and 80 days in the longest fever. In most cases, black tongue coating appears 2 weeks after fever, with an average of about 20 days. It can be seen that the longer the fever lasts, the more black tongue coating appears. Among patients with black tongue coating, half of the cases are caused by various inflammatory infections, including pneumonia, pyelonephritis, gangrenous appendicitis, peritonitis, cholecystitis, lower limb phlebitis, suppurative osteomyelitis, pelvic inflammatory disease and sepsis. Infection, high fever, toxin stimulation and other factors can cause the filiform papillae of the tongue to proliferate excessively, and the staining of microorganisms turns them black.

Second, gastrointestinal dysfunction:

Clinical observations show that more than half of the patients with black tongue coating have gastrointestinal symptoms such as abdominal distension, constipation, nausea, and poor appetite. After treatment with purgative agents, the black tongue coating will disappear if bowel movements become smooth and internal heat subsides. After the black tongue coating disappears, appetite will also improve. These symptoms of gastrointestinal dysfunction are mostly seen in patients with high fever, so intestinal dysfunction may be one of the symptoms of poisoning. Gastrointestinal dysfunction caused by the stimulation of bacterial toxins reduces the pH value of oral saliva, increases the adhesion between tongue cells, and lengthens the keratin protrusions of the filiform papillae, which are easily stained black by microorganisms to form black fur.

Third, fungal infection:

In the human mouth (including on the tongue coating), there grow various kinds of bacteria, molds and other microorganisms. They usually restrict and promote each other, maintaining a certain balance, and do not hinder human health. If a large number of broad-spectrum antibiotics are used to treat diseases, the balance of these microorganisms will be broken. Some bacteria that are more sensitive to antibiotics are killed, while fungi that are relatively resistant to these antibiotics take the opportunity to multiply rapidly. Since molds can produce a variety of colors, the tongue can appear in a variety of colors, ranging from brown to black.

Fourth, central nervous system dysfunction:

A patient went to the hospital for esophageal barium radiography because of esophageal swallowing obstruction. A small nodule was found on the esophageal wall, and esophageal cancer was suspected. As a result, the patient became listless, depressed, and burdened with thoughts. He could not eat or sleep, and a black, brush-like tongue coating appeared on his tongue. After another esophageal barium angiography and radiography, and consultation with doctors from various departments, the diagnosis of esophageal cancer was denied. His mental state returned to normal, no drug treatment was used, and the black hairy tongue gradually subsided. It is common to see cases of black hairy tongue due to central nervous system dysfunction caused by mental stress. This is because when the central nervous system is dysfunctional, it can cause increased acidity in the oral cavity, which is conducive to the growth of color-producing mold and causes the appearance of black tongue. Fifth, some critically ill patients:

Certain chronic diseases and visceral failure, such as severe liver damage in late-stage liver cirrhosis, chronic renal failure and uremia, and various advanced cancers with extremely weak physical constitution.
