If the color of semen is red, it means there is blood in the semen; if the semen is very yellow and thick, men need to consider pus cells at this time. It is normal for men to have yellow semen when they have not ejaculated for a long time. If there is inflammation in the male reproductive tract, a large number of pus balls will be found under a microscope.

What does semen look like?

1. Semen volume : normal ≥2ml. If it is greater than 7ml, it is too much. Not only does the sperm density decrease, but it is also easy to flow out of the vagina, resulting in a decrease in the total sperm count, which is common in seminal vesiculitis. If it is less than 2ml, it is too little semen, but usually less than 1ml is too little. At this time, the contact area between semen and the female reproductive tract is small, or the viscosity makes it difficult for sperm to enter the female's cervix, leading to infertility. This is common in severe accessory gland inflammation, low testosterone levels, ejaculatory duct obstruction, retrograde ejaculation, etc.

2. Color : Normally off-white or slightly yellow. Milky white or yellow-green color indicates inflammation of the reproductive tract or accessory glands; pink, red, or red blood cells visible under a microscope are bloody semen, which is common in inflammation of the accessory glands and posterior urethra, and occasionally seen in tuberculosis or tumors.

3. Acidity and alkalinity : The normal pH value of semen is 72-78. A value less than 7.2 is seen in ejaculatory duct obstruction or urine contamination; a value greater than 7.8 is seen in seminal vesiculitis or old specimens.

4. Liquefaction time : After normal semen is ejaculated, it becomes a jelly under the action of the seminal vesicle coagulase, and after 15 to 30 minutes it becomes a liquid under the action of the prostate liquefaction enzyme. This is semen liquefaction. It is abnormal if semen has not liquefied 30 minutes after ejaculation.

5. Viscosity : Place a glass rod in contact with liquefied semen and lift it gently to form a semen thread, which is normally less than 2 cm in length.

6. Sperm count : generally expressed as the number of sperm per milliliter of semen. Normal count ≥20×106/ml. A value lower than this means there are too few sperm, which is seen in spermatogenesis disorders caused by various reasons. It may cause subfertility or infertility due to reduced chances of sperm entering the uterine cavity and fallopian tubes. If the sperm count is greater than 250×106/ml, it means there are too many sperm, which can also lead to infertility because their motility is affected.

7. Sperm morphology : Sperm with normal morphology ≥ 50%, otherwise it may cause infertility.

8. Motility : ≥50% of sperm move forward rapidly in a straight line.

9. Survival rate : usually refers to the examination within 1 hour after ejaculation, and the live sperm ≥50%. Common causes of decreased sperm motility and survival rate include inflammation of accessory glands, varicocele, ciliary hypoplasia syndrome caused by chronic respiratory tract infection, the presence of antisperm antibodies in semen, or improper specimen storage.

10. White blood cells : The white blood cells in normal semen are <1×106/ml. Leukocytosis suggests infection of the reproductive tract or accessory glands.

How to check sperm yourself

1. Semen volume : The normal amount should be more than or equal to 2 ml (milliliters). If it is more than 7ml, it is too much. At this time, not only the sperm density becomes low, but it is also easy to flow out of the female vagina, resulting in a decrease in the total sperm count. This is common in patients with seminal vesiculitis. If the total amount is less than 2ml, it means the semen volume is small. If it is less than 1ml, it is too little and can easily lead to infertility.

2. Color : The normal color is off-white or slightly yellow. If the semen appears milky white or yellow-green, it indicates inflammation of the male reproductive tract or accessory glands. If it appears pink or red, or red blood cells can be seen under a microscope, it is bloody semen, which is common in patients with accessory glands and posterior urethritis, and occasionally in patients with tuberculosis or tumors.

3. Liquefaction time : After normal semen is ejaculated, it will become a jelly under the action of the seminal vesicle coagulase. After 15-30 minutes, it will become a liquid under the action of the prostate liquefaction enzyme, which is called semen liquefaction. If semen does not liquefy 30 minutes after ejaculation, it is considered abnormal.

4. Viscosity : Use a glass rod to touch the liquefied semen and gently lift it up to form semen threads. If the semen is normal, the length of the semen thread should be less than 2 cm.

In addition, indicators such as semen pH and sperm motility need to be tested to obtain accurate results. If men find that the above four indicators are not normal, they should go to the hospital for further laboratory tests.
