Chronic injuries cause more urinary abnormalities than acute injuries. The injured parts are mostly in the lower waist and buttocks, followed by the upper waist and cervical sections. 1. Applied anatomy: The blood supply to the brain mainly comes from the internal carotid artery and the vertebral artery. Below the second lumbar vertebral canal is the cauda equina nerve, which is involved in regulating urination and defecation. Its innervation is saddle-shaped. The sacrum is a low-level parasympathetic nerve center that is involved in regulating urination. The pudendal nerve passes through the inferior foramen of the piriformis muscle and is distributed to the pudendum. The above anatomical characteristics are all directly related to urination.

2. Etiology and pathology : Trauma, strain and other reasons may cause damage to the spinal joints and surrounding soft tissues, which may stimulate related tissues and lead to abnormal urination. If the trauma occurs in the cervical spine and affects the vertebral artery, it may cause ischemia of the urination center in the brain. If it occurs from the 12th thoracic vertebra to the 2nd lumbar vertebra and damages the lumbar vertebra, it may cause dysfunction of the bladder's autonomous urination center. If it occurs in the lower lumbar region, it may affect the normal function of the cauda equina. If it occurs in the sacrum, it affects the parasympathetic nerves. If it occurs in the piriformis muscle, the pudendal nerve may be affected due to piriformis muscle spasm or inflammation. The above reasons may affect urination and cause abnormal urination. From the perspective of traditional Chinese medicine, the spine is the pathway of the Du meridian, and its two sides are the pathways of the bladder meridian. Damage to the Du meridian and bladder meridian, blockage of the meridians, and dysfunction of qi transformation can also lead to abnormal urination.

3. Clinical manifestations and diagnosis : Abnormal urination caused by soft tissue injuries of the spine and buttocks is mostly functional and occurs at all ages, but mostly in teenagers or young adults. The clinical features are as follows :

(1) History of trauma or strain on the spine or hips.

(2) Abnormal urination is related to damage to the corresponding parts of the spine or buttocks; at the same time, there is swelling and pain in the above-mentioned parts and limited function. When the cauda equina is damaged, numbness of the saddle area may also occur.

(3) Symptoms of abnormal urination include urgency, frequency, less urination, more urination, difficulty controlling urination, or nocturnal enuresis. If the condition does not heal for a long time, it may lead to urinary tract infection.

(4) Urine examination generally shows no abnormalities. In severe cases, routine urine examination may reveal white blood cells, pus cells, etc.
