Differential Diagnosis

1. Depression patients suffer from low mood and slow thinking, and on this basis they develop a sense of unreality and nihilistic delusions. The patient complains that things around them seem "unreal", "unnatural", and "emotionless". In severe cases, they deny the existence of themselves and the world around them. Adding other symptoms of depression does not diagnose this disorder.

2. Menopausal depression may also cause nihilistic delusions, often believing that one's brain is empty, nerves are broken, and have a sense of unreality. This disease occurs during menopause, and its clinical symptoms are characterized by emotional depression, anxiety and tension, accompanied by symptoms such as autonomic nervous system and endocrine dysfunction.

3. Schizophrenia may also cause nihilistic delusions, often believing that one's organs are gone, that things around them no longer exist, and other symptoms of schizophrenia.

4. Paralytic dementia may also cause nihilistic delusions, but other symptoms are more prominent, such as mental retardation, emotional indifference, personality disorders, etc.
