Gastric bleeding is not uncommon in life, and this problem can pose a great danger to our health, so we must pay attention to a comprehensive examination, which includes laboratory tests and special examinations including x-rays or radionuclide scans. 1. Laboratory tests: In case of gastric bleeding, key tests should include routine blood tests, blood type, clotting time, occult blood test in stool or vomitus (radionuclide or immunological occult blood test can be performed if conditions permit), liver function tests and blood creatinine and urea nitrogen tests, etc. If conditions permit, the hematocrit should be measured. 2. Special inspections: 1. X-ray barium contrast imaging: Although the diagnostic value of endoscopic examination is superior to that of X-ray barium contrast imaging, it cannot replace it. Because some anatomical parts of the intestine cannot be seen by ordinary endoscopes, and due to the lack of experience of some endoscopists, lesions are sometimes missed, which can be remedied by X-ray barium examination. However, barium angiography should not be performed too early after active bleeding, otherwise it may cause rebleeding or aggravate bleeding due to pressing the abdomen. It is generally recommended to perform the procedure 3 days after bleeding stops and the condition stabilizes. 2. Selective arteriography: It is a supplementary examination when the above diagnosis cannot be confirmed, and it is more valuable for the diagnosis of vascular malformations. 3. Thread-containing capsule test: It has certain value in locating bleeding caused by lesions in the distal duodenum and proximal jejunum. 4. Radionuclide scanning: Radionuclide scanning can be performed for cases with negative results by endoscopy and X-ray examinations. The method is to use radionuclides (such as 99mtechnetium) to label the patient's red blood cells and then inject them into the patient's body through the vein. When there is active bleeding and the rate can reach 0.1ml/min, the radionuclide can show the bleeding site and the monitoring time can be up to 24 hours. Experience has shown that if this test is negative, selective arteriography is often also negative. Diagnostic method for gastric bleeding: Gastric juice analysis: Gastric juice analysis refers to the extraction of gastric juice for the measurement and examination of relevant indicators to determine whether the gastric juice is normal. It mainly includes general property inspection, chemical inspection and microscopic inspection. |
