How to care for tarsal dysfunction?

Patients with tarsal dysfunction must pay attention to correct treatment and care, such as taking some antibiotics orally, but they must consult a doctor for drug treatment and pay attention to correct care. They can use hot eyelids to relieve symptoms.

1. Physical cleaning of eyelids: Pay attention to eyelid hygiene. When the meibomian glands are blocked, apply hot compresses to the eyelids for 5 to 10 minutes to soften the meibomian gland secretions. Then place your fingers on the eyelid skin surface relative to the meibomian glands, rotating and pushing toward the eyelid margin to drain the secretions. You can use non-irritating shampoo or special liquid medicine such as boric acid solution to clean the local eyelid margin and eyelashes. Cleansing the eyelids in the morning is more effective due to the accumulation of scales at night.

2. Oral antibiotics: doxycycline 50 mg orally, 2 times a day. It needs to be taken continuously for several weeks to take effect and needs to be maintained for several months. Common side effects include photosensitivity and abnormalities in tooth enamel, so it should be used with caution in children under 8 years old, pregnant women, and breastfeeding women.

3. Application of topical medications: including antibiotic eye drops for the treatment of blepharitis, short-term glucocorticoid eye drops, and preservative-free artificial tears. Topical 1% metronidazole cream or 1% clindamycin lotion are effective in controlling infection of the facial skin in rosacea. If the patient has seborrheic dermatitis, the scalp skin can be cleaned with a shampoo containing an antiseborrheic drug such as selenium sulfide or tar. In recent years, topical androgens have been tried to treat MGD. 3% testosterone oil strips, 10 mm long and 2 mm wide, are placed in the upper and lower fornixes of the conjunctiva 3 times a day. After 2 to 3 months, symptoms, tear film lipid layer thickness, and tear film breakup time are significantly improved. The long-term efficacy needs further observation.

4. Hot compress and massage: Use any hot object (such as a wet towel heated in a microwave, an electric heating pad, a bottle of hot water, etc.) to apply hot compress to the eyes for three minutes, then massage several acupoints around the eyes (Sibai, Chengqi, Zanzhu, Jingming, Yuyao), especially in the middle of the upper eyelid, squeezing from top to bottom. Do it once when your eyes are tired, and once before going to bed and after waking up in the morning. It has obvious therapeutic effect on dry eye caused by meibomian gland secretion disorder (short tear film breakup time). Do it when you have time, the more the better.
