There are many reasons that can cause ovarian pain, such as pelvic inflammatory disease, water accumulation, corpus luteum rupture, ovarian cysts, etc., which can all lead to the occurrence of this symptom. In addition, ovarian pedicle torsion is also one of the reasons, but the possibility of this happening is relatively small.

1. The mature ovarian follicle or corpus luteum may be damaged and bleed due to some reasons. In severe cases, it may cause massive intra-abdominal bleeding. It may be caused by spontaneous rupture, pelvic inflammation, acute or chronic appendicitis causing ovarian congestion; infectious diseases, anemia, malnutrition or other conditions before menstruation causing partial damage to platelets and changes in blood components, leading to bleeding. Or rupture may occur directly or indirectly due to external forces, such as sexual intercourse or increased intra-abdominal pressure.

2. The time of ovarian rupture has a certain relationship with the menstrual cycle and can be used as the main basis for diagnosis. About 80% of ovarian ruptures are caused by rupture of the corpus luteum or corpus luteum cyst, so it usually occurs after ovulation, mostly in the last week of the menstrual cycle, and occasionally on the second day of the menstrual period. In a small number of cases, the disease is caused by follicle rupture, which often occurs in mature follicles, so the onset usually occurs between the tenth and eighteenth days of the menstrual cycle. Patients with ovarian rupture generally have no history of ovarian dysfunction, and most of them have ovulatory cycles. Abdominal tenderness is not obvious, but bimanual examination reveals very obvious pelvic tenderness, which, combined with the menstrual history, can usually lead to a diagnosis.

3. The general symptoms of ovarian cyst pedicle torsion include ovarian cyst, sudden severe and persistent right lower abdominal pain, which is more severe than appendicitis. It can be diagnosed through pelvic examination or B-ultrasound. Severe pain may be accompanied by nausea, vomiting, and even shock. Torsion of the pedicle can cause congestion and even rupture of blood vessels and bleeding. Once diagnosed, the cyst should be removed surgically immediately, otherwise serious consequences may occur.
