2. Classification by degree of increase in pulmonary artery pressure:

Mild pulmonary hypertension: pulmonary artery systolic pressure 30-40 mmHg, Pp/Ps>0.45, pulmonary vascular resistance 251-500 dyne. sec. cm-5. Moderate pulmonary hypertension: pulmonary artery systolic pressure 40-70 mmHg, Pp/Ps 0.45-0.75, pulmonary vascular resistance 500-1000 dyne. sec. cm-5. Severe pulmonary hypertension: pulmonary artery systolic pressure>70 mmHg, Pp/Ps>0.75, pulmonary vascular resistance>1000 dyne. sec. cm-5.80 dyne. sec. cm-5=1 Wood

3. According to total lung resistance, it is divided into:

Normal whole lung resistance is less than 20 to 300 dyne.s.cm5 or 2.5 to 3 Wood units.

When the total lung resistance is slightly elevated, it is 300 to 400 dyne.s.cm5 or 3.75 to 5 Wood units.

When the total lung resistance is significantly increased, it is ≥450 dyne.s.cm5 or 5.6 Wood units.

Pathological grading

Stage 1 (Grade A): There is abnormal expansion of muscle tissue into the peripheral arterioles and mild thickening of the normal muscular blood vessels, with the wall thickness being less than 1.5 times that of normal. This change is caused by chronic high blood flow and high pressure stretching the blood vessel wall, which causes the regeneration and hypertrophy of smooth muscle.

Stage II (Grade B): Further growth and hypertrophy of muscle tissue: In the mild stage II, the middle wall thickness is 1.5 to 2 times that of normal, and the pulmonary artery pressure has increased. In the severe second stage, the middle wall thickness is more than twice that of normal, and the pulmonary artery pressure reaches half of the systemic circulation pressure. The vessel wall thickens due to myocyte hypertrophy, myocyte proliferation, and increase in extracellular matrix proteins.
