Gastric leiomyoma is a common type of fibroid disease. Most gastric leiomyoma are benign, but they also need to be treated in a timely manner. When gastric leiomyoma appears, the symptoms are not very obvious, but they can still be distinguished through careful examination. Clinical manifestations Gastric leiomyoma is a benign tumor that originates from smooth muscle tissue (mostly from the circular muscle or longitudinal muscle of the gastric wall), and a few originate from the muscularis mucosa. It is common in the gastric fundus and body, more common on the lesser curvature than the greater curvature, and more common on the posterior wall than the anterior wall. Leiomyomas less than 2 cm in diameter have no clinical symptoms. Its clinical manifestations are often related to the location, size, growth pattern, and type of complications of the tumor. In severe cases, the main manifestations are bleeding, abdominal pain, abdominal distension, abdominal mass, etc., among which bleeding is the most common symptom. examine 1. Routine inspection There may be a decrease in red blood cell count and hemoglobin, indicating the presence of anemia. 2. Fecal occult blood test The digestive tract often presents intermittent small amounts of bleeding, manifested as a positive fecal occult blood test or black stools. 3. Histopathological examination The tumor has clear boundaries and no capsule; microscopy shows well-differentiated spindle cells interlaced in bundles with varying amounts of fibrous connective tissue and fibroblasts. 4. X-ray examination The typical signs of upper gastrointestinal tract barium meal are a tumor protruding into the gastric cavity, a round or oval filling defect in the stomach with a neat shape and clear edges, normal surrounding mucosa and gastric wall, and no peristaltic dysfunction. In patients with tumors complicated by ulcers, a deep niche shadow with smooth surroundings and no mucosal accumulation can often be seen in the filling defect area formed by the tumor. When a subserosal tumor or a tumor protrudes outside the stomach, the traction and compression of the tumor may cause the stomach wall to become deformed or present as an external pressure mark-like defect. 5. Abdominal ultrasound examination Ultrasound examination using the drinking water method can clearly show the location, size, growth pattern and internal echo structure of the tumor, which is of certain value in diagnosing gastric leiomyoma. If combined with gastroscopy and biopsy, the diagnosis rate can be greatly improved. Ultrasound imaging showed that the leiomyoma had smooth edges and uniform low echoes inside. |
