The brainstem was once considered a forbidden area for neurosurgery, but due to the development of microneurosurgery in the past decade, brainstem surgery has been widely performed. For brainstem hemorrhage, we only need to look at the amount and location of bleeding. If the bleeding is very small, no surgery is needed. If the bleeding is too much, there is no value in treatment. We must make a decision by looking at the CT scan. Brainstem hemorrhage may also cause hydrocephalus requiring ventricular drainage, and deep coma may require tracheotomy.

The brainstem area has always been listed as a surgical restricted area by neurosurgery and it cannot be touched. The reason is that the brainstem is the center of human respiration and circulation. If invaded, the patient will experience changes in breathing and circulation. However, with the rapid development of microscopy technology in the past decade or so, many hospitals in China have made great achievements in the treatment of brainstem lesions, which has led to significant development of brainstem surgery. Therefore, patients with brainstem hemorrhage can reasonably choose the surgical method according to the amount and location of bleeding. Specific surgical methods include directional puncture and drainage, microscopic hematoma removal, and neuroendoscopic hematoma removal, which are all methods for treating brainstem hemorrhage.

In the case of brainstem hemorrhage, the treatment is currently mainly individualized, depending on the severity of the bleeding. Some brainstem hemorrhages may be able to achieve the desired results with conservative treatment. Some cases of brainstem hemorrhage may require more drastic measures, such as direct craniotomy to remove the hematoma. Some cases of brainstem hemorrhage may require minimally invasive methods, such as stereotactic soft channel catheterization, or simple brain puncture and catheterization, etc. However, no matter which treatment is used, after surgery, long-term rehabilitation and physical therapy training, supportive treatment and other conditions may be required.
