Generally speaking, patent ductus arteriosus surgery is usually an acute treatment for congenital heart disease, and this treatment method is a relatively simple one for treating congenital heart disease. Some patients undergo this surgery about two years after birth. The reason why this operation needs to be performed in about two years is that after the birth of a newborn, the ductus arteriosus can close on its own, generally within two weeks to two years after birth. If it does not close, this operation will be considered. Once the diagnosis of patent ductus arteriosus is confirmed, surgery can be performed if there are no contraindications. The appropriate age for surgery is 2 to 6 years old. Preoperative preparation: Detailed physical examination and related examinations to confirm the diagnosis and rule out other heart malformations. If heart failure occurs, it should be corrected first; if bacterial endocarditis is present, surgery should be performed 2 to 3 months after the inflammation is controlled. Body position: Left side lying. Medicine is all available online. Anesthesia: General anesthesia with endotracheal intubation. Surgical steps: Arterial duct ligation is suitable for young children with slender ducts, soft and elastic walls, and no bacterial infection. Incision: Left posterolateral thoracotomy. Enter the chest through the 4th or 5th intercostal space. The left lower lobe was pulled forward and downward to expose the ductal triangle formed by the left pulmonary artery, phrenic nerve and vagus nerve (Figure 1). A continuous tremor may be palpated in this area. By cutting the mediastinal pleura longitudinally between the phrenic nerve and the vagus nerve, the duct between the aorta and the pulmonary artery can be exposed. Blocking bands are placed on the aorta at the upper and lower ends of the free catheter. Carefully separate the front and upper and lower edges of the catheter, and then bluntly separate its posterior wall. Avoid injuring the left recurrent laryngeal nerve during the operation. Allow sufficient catheter length for ligation. Patent ductus arteriosus surgery is a relatively simple operation. The specific surgical procedure of this operation generally needs to be determined based on the patient's specific condition. The above introduction is only the common surgical process. In addition, patent ductus arteriosus surgery will not affect the patient's future life, but during the operation, it is best to follow up for one year. If no abnormalities occur after one year, then there is no need for another surgery in the future. |
