Causes

Enterovirus is mainly transmitted through the fecal-oral route, and a few are transmitted through respiratory secretions. Most viruses cause initial infection in the lower gastrointestinal tract. There are special receptors on intestinal cells that bind to enterovirus. The virus passes through the intestinal blood, produces viremia, and then enters the cerebrospinal fluid through the choroid plexus to invade the meninges, causing inflammatory changes in the meninges.

Clinical manifestations:

1. The disease is most prevalent in summer and autumn, but can occur all year round in tropical and subtropical areas. It is more common in children, but adults can also suffer from it. Most of the cases have an acute onset and are characterized by systemic poisoning symptoms of viral infection such as fever, headache, photophobia, myalgia, nausea, vomiting, loss of appetite, diarrhea, and general fatigue, and may also have signs of meningeal irritation. The course of the disease usually lasts more than 1 week in children and 2 weeks or longer in adults.

2. Clinical manifestations may vary depending on the patient's age, immune status, and virus type and subtype. For example, young children may experience fever, vomiting, rash, etc., while neck stiffness is mild or even absent. Hand-foot-mouth syndrome often occurs in enterovirus 71 meningitis, and nonspecific rash is common in echovirus 9 meningitis.
