Endometrial cancer, also known as uterine body cancer, is a malignant tumor that occurs in the endometrium. The main pathological type is adenocarcinoma, followed by adenocarcinoma and adenosquamous carcinoma. Their clinical manifestations are basically similar. The most common symptom is vaginal bleeding after menopause, and premenopausal women often show menstrual disorders. What is the mid-term recurrence rate of endometrial cancer? To investigate the recurrence characteristics and related factors of advanced endometrial carcinoma. The clinical and pathological data of 20 patients with recurrent advanced endometrial carcinoma admitted between August 1989 and May 2001 were collected and compared with those of patients without recurrence during the same period. ① 60 patients with advanced endometrial cancer were followed up for 1 to 7 years, and 20 cases of recurrence and metastasis were found, with a recurrence rate of 33%, including 13 cases of stage II and 7 cases of stage III; ② The first site of recurrence and metastasis: pelvic and abdominal cavity in 9 cases, lung metastasis in 8 cases, and other sites in 3 cases; ③Clinical characteristics: The recurrence rates of stage c and stage a were 40%, 63%, and 63%, respectively, which were significantly higher than those of stage a (p<0.05); the recurrence rate of patients with residual lesions after surgery was 72%, while that of patients without residual lesions was only 24%, and the difference was statistically significant (p<0.05). ④Pathological characteristics: The recurrence rate was 50% in cases where the muscle was invaded ≥1/2, 18% in cases where the muscle was invaded <1/2, 58% in cases where the paracervix was invaded, and 27% in cases where the paracervix was not invaded. The differences were statistically significant (p<0.05). The recurrence rate of poorly differentiated cancer was 55%, which was significantly higher than that of moderately differentiated cancer (25%) and well-differentiated cancer (20%) (p<0.05). The recurrence rate of lymph node metastasis (46%) and that of lymph node without metastasis (23%) were not significantly different (p>0.05). ⑤ Recurrence and treatment methods: Postoperative adjuvant chemotherapy and radiotherapy have the lowest recurrence rate, followed by postoperative adjuvant chemotherapy. Conclusion The recurrence rate of advanced endometrial cancer is high. The most common sites of recurrence and metastasis are the pelvic and abdominal cavity and lungs. Most recurrences occur within 2 years. The high-risk factors for recurrence and metastasis include tumor stage, muscle invasion depth, tumor differentiation degree, paracervical invasion, and the presence or absence of residual lesions. The treatment should be based on surgery, with postoperative adjuvant chemotherapy and radiotherapy. |
