⒈Hepatic encephalopathy is usually a complication of liver cancer in the terminal stage. It is caused by extensive and severe damage to the liver parenchyma caused by liver cancer or concurrent cirrhosis. The prognosis of liver cancer with hepatic encephalopathy is much more serious than that of hepatic encephalopathy complicated by other liver diseases. Drugs that damage the liver, bleeding, infection, electrolyte imbalance, the use of large amounts of diuretics or paracentesis are often factors that induce hepatic encephalopathy.

⒉ Gastrointestinal bleeding is mostly caused by portal hypertension due to liver cirrhosis or cancer thrombus, which causes esophageal varicose veins to rupture and bleed. Patients often die from hemorrhagic shock or induced hepatic encephalopathy. In addition, patients with advanced liver cancer may also suffer from extensive bleeding due to gastrointestinal mucosal erosion, ulcers and coagulation disorders.

⒊ Other complications Primary liver cancer is prone to various infections due to long-term consumption, weakened body resistance or long-term bed rest, especially in the case of chemotherapy or radiotherapy caused by decreased white blood cells, it is more likely to have complications such as pneumonia, sepsis, intestinal and fungal infections. Liver cancer close to the diaphragm can directly infiltrate, or metastasize through the lymphatic and blood routes to cause bloody pleural effusion. Bloody ascites may also occur due to cancer rupture or direct infiltration and dissemination to the abdominal cavity. In addition, some people have reported cases of successful surgical removal of right atrial cancer thrombus obstruction caused by hepatic vein cancer thrombus. Ehrich reported a case of acute right atrial cancer thrombus obstruction. The cancer thrombus removed by surgery was approximately 8cm×6cm×10cm, and the pathology report was a metastatic cancer thrombus of primary hepatocellular carcinoma.

⒋ There is also rupture and bleeding of cancerous nodules. Liver cancer can rupture spontaneously due to tumor development, necrosis and softening, or due to external force, increased intra-abdominal pressure (such as severe coughing, straining to defecate, etc.) or after a physical examination. The chance of rupture of massive liver cancer is more common than that of nodular liver cancer. When liver cancer ruptures, patients have severe abdominal pain, abdominal distension and cold sweats, and shock may occur in severe cases. The amount of internal bleeding caused by liver cancer is small due to the small rupture, which can often be stopped by the adhesion of the greater omentum, and the symptoms can be relieved by themselves after 3 to 5 days. During the physical examination, tenderness, rebound pain and muscle tension can be found in the abdomen. In severe cases, the pulse is thin and rapid, blood pressure is low, the abdomen is distended, and there is mobile dullness. Heavy bleeding caused by liver cancer rupture can lead to the death of the patient in a short period of time. If the bleeding is stopped by surgery, some patients can prolong their lives. There are also long-term survivors who have surgical resection of early small cancer nodules.
