Is there any way to treat pancreatic tumors?

Pancreatic tumors lack obvious symptoms in the early stages, and most cases are diagnosed in the late stages, making surgical resection less likely. Surgical treatment requires different surgical methods based on the different stages of the disease and the extent of local invasion of the tumor lesions.

1. Pancreaticoduodenectomy: If the tumor is located in the head of the pancreas, there is no metastasis to the porta hepatis, around the celiac artery, at the root of the mesentery, or at distant lymph nodes, there is no invasion of the hepatic artery, superior mesenteric artery, or inferior vena cava, there is no invasion or only partial invasion of the portal vein, and there is no organ metastasis, then pancreaticoduodenectomy can be performed.

2. Pylorus-preserving pancreatoduodenectomy. The main conditions for the implementation of pylorus-preserving pancreatoduodenectomy for pancreatic head cancer are: ① The lesion has not yet invaded the pylorus and duodenal bulb. ② There is no pyloric lymph node metastasis. In addition, pylorus-preserving pancreatoduodenectomy can also be performed for pancreatic head tumors with low malignancy (cystadenocarcinoma, islet cell carcinoma, acinar cell carcinoma, etc.).

3. Pancreatic tumor surgery combined with vascular resection. In the past, it was believed that invasion of the portal vein and superior mesenteric vein system by tumor was a contraindication to surgical resection. This is one of the methods for treating pancreatic tumors. However, the surgical resection rate is relatively low. In the past 20 years, with the strengthening of preoperative and postoperative treatment and the improvement of surgical operation technology, tumor invasion of the portal vein system is no longer a contraindication to surgical resection. This method has significantly improved the surgical resection rate of pancreatic tumors or periampullary cancer.

4. Tail pancreatectomy: Tail pancreatectomy is a commonly used method for treating pancreatic tail tumors and is suitable for pancreatic tail cancer without distant metastasis.

5. Total pancreatectomy: Patients with pancreatic head cancer who are under 65 years old and whose surgical exploration shows that the lesion is stage I or II (without lymph node metastasis) can undergo total pancreatectomy.

6. Surgical treatment for unresectable pancreatic tumors: bile duct drainage, external drainage for bile duct obstruction. For the elderly, the weak, those with hypoproteinemia, bile duct infection, or those with poor liver and kidney function who cannot tolerate internal bile duct drainage, bile duct or cholecystostomy external drainage can be performed. The operation is simple and has few complications. Cholecystoduodenostomy or bile duct anastomosis has the advantages of simple operation, less trauma, and fewer surgical complications. However, ascending infection is prone to occur after the operation, and it is rarely used in clinical practice.
