Uremia, also known as chronic renal failure, has very complex pathogenic factors and is very harmful. It is often not an independent disease, but a syndrome of various late-stage kidney diseases. Early treatment of uremia is very important. When suspected symptoms are found, timely examination and diagnosis are needed, but many people do not know much about the examination methods for uremia. So, can uremia be detected through ultrasound? Let’s take a look below. The normal structure of the kidneys of patients with uremia has been mostly destroyed, replaced by the proliferation of matrix in the glomerular mesangial region, so that too much matrix invades the normal tissue, causing hardening of the renal tissue. There is also proliferation of fibrous tissue in the renal interstitium, atrophy and collapse of the renal tubules, causing the entire kidney to shrink, harden and atrophy. Sun Jifeng, Department of Nephrology, Xi'an Tangdu Hospital Ultrasound cannot directly detect uremia. Uremia is usually diagnosed by testing urine for creatinine, protein, etc. B-ultrasound can only be used to check whether there are obvious lesions in the kidneys to indirectly prove whether there are signs of uremia. Specific inspections can be divided into the following categories: 1. When the blood routine examination for uremia is performed, the hemoglobin is generally below 80g/L, and most of the time it is only 40-60g/L, which is normocytic normochromic anemia. When the patient has combined chronic blood loss and malnutrition, it may also manifest as microcytic hypochromic anemia. 2. Urinalysis examination shows that the changes in urine of patients with uremia vary greatly depending on the primary disease. The urine osmotic pressure decreases, the specific gravity is lower, and the urine volume decreases. In the late stage, when the creatinine clearance rate drops to below 1.0-2.0 ml/second, anuria may occur. The urine protein is +~+++. In the late stage, because most of the glomeruli have been destroyed, the urine protein decreases instead. If the urine sediment examination can find thick, short, homogeneous, waxy casts with cracked edges, it will be helpful for diagnosis. 3. Renal function test: in the uremia stage, when the renal creatinine clearance rate is <25ml/min, the blood creatinine will increase significantly, accompanied by metabolic acidosis. 4. Blood biochemistry examination showed decreased plasma protein, with total protein often below 60g/L, among which the decrease in albumin was often more obvious, mostly below 30g/L. Blood calcium is low, usually around 2mmol/L, and blood phosphorus is usually higher than 1.7mmol/L. Blood potassium and blood sodium depend on the condition. |
