Many people don’t know what dental piling is. This type of repair mainly refers to large tooth defects. What is a pile?

The post is the part inserted into the root canal, which uses friction, adhesion and bonding forces to achieve retention, thereby providing retention for the final restoration.

What is the function?

The main function is retention, followed by stress transfer and changing the stress distribution of the tooth root.

Length of pile?

Ensure that the root filling material of more than 4mm isolates the oral cavity and the periapical area;

Ensure that the length of the post is not shorter than the clinical crown length;

The length of the post is greater than 1/2 of the length of the root supported by the alveolar bone, that is, the length of the intrabone post is greater than 1/2 of the length of the intrabone root. In this way, the end of the post will not be too close to the alveolar ridge, greatly reducing root fracture.

Pile diameter?

Generally speaking, under the condition that the strength of the post material is sufficient, the diameter of the post in the range of 1/4 to 1/3 of the root diameter has no obvious effect on the fracture resistance of the tooth root.

Dentin shoulder collar

In principle, there is at least 1.5 mm of dentin between the edge of the core and the edge of the crown. When the root surface is located below the gingiva, orthodontic root traction or periodontal crown lengthening is required to obtain sufficient dentin shoulder. At the same time, ensure that the thickness of the tooth at the dentin shoulder is not less than 1mm and the height is not less than 1.5mm.

What kind of dental conditions require posting?

1. Those with large-area tooth crown defects due to caries (cavities), trauma, etc.;

2. The tooth is missing and only the root is left. The root can still reach under the gums using the residual root (no crown). The periodontal health is good and the root is long enough to expose the root surface through gingivectomy.

3. Those with misaligned or twisted teeth who are unable to undergo orthodontic treatment or are not suitable for orthodontics; 4. Those with deformed teeth or poor retention of direct molars.

What material is best for dental pile driving?

There are many materials used for dental posting, the most commonly used ones are fiber posts and metal posts. The materials of metal piles are divided into stainless steel, gold-palladium alloy, gold-platinum alloy, etc.

If it is the front teeth, then it is more appropriate to use fiber posts (understand the price and shelf life of fiber posts). It is corrosion-resistant, does not contain metal, does not affect medical examinations, is not prone to root fractures, has good aesthetic properties, and can be used in conjunction with all-ceramic teeth. If it is the back teeth, it is more appropriate to use metal post. Because it is located at the back teeth, the requirements for aesthetics do not need to be particularly high, and metal post will not cause root fracture and is corrosion-resistant.

Precautions

Piling requires caution. After root canal treatment, piling is likely to irritate the original trauma and cause painful root cracks. So pay attention to the following:

1. It is generally recommended that patients observe for a period of time after treatment, and then proceed with pile driving and crown making if there is no discomfort (in case of lateral root canals or incomplete treatment).

2. Teeth that have been treated are generally more brittle, so metal posts are not recommended. Fiber posts can be selected to reduce the chance of root cracks.

3. If your teeth have been injured, be sure to tell the doctor about it during treatment so that the doctor can understand the overall condition of the teeth.
