Nursing points for primary care

We all know that not everyone can be a hospital nurse. They have to go through a series of exams and a series of practical activities before they can enter the hospital. Because nursing is not as simple as caring for patients in our phenomena, but requires different nursing methods based on the needs of different patients. Nursing is also divided into different levels. What are the key points of first-level nursing?

The care of primary care patients includes the following elements:

1. Check on patients every hour and observe changes in their conditions;

(2) Measure vital signs according to the patient's condition;

(3) Correctly implement treatment and medication measures according to the doctor's instructions;

(IV) According to the patient's condition, correctly implement basic nursing and specialized nursing, such as oral care, pressure sore care, airway care and tube care, and implement safety measures;

(V) Provide health guidance related to nursing.

Level 1 care is marked in pink, indicating intensive care but without dedicated staff. For the vast majority of critically ill patients, this is considered a high level of care. According to regulations, for patients receiving first-level care, nurses should make rounds every hour to understand their condition and treatment, and help with their diet and daily life. According to the patient's condition, help the patient change body position, bathe, wash hair, cut nails, etc.

There are four levels of care, namely special care (special care), primary care, secondary care and tertiary care (ordinary care). After the patient is admitted to the hospital, the doctor will determine the level of care based on the patient's condition, issue medical orders, and set different marks on the inpatient list and the patient's bedside card to remind the nurses to implement the orders and marks, and the head nurse will supervise and inspect. Let’s talk about the different requirements for different levels of care.

1. Special care (special nursing) is marked in bright red. Patients with critical conditions or after major surgery may have accidents at any time and need close observation and intensive care. Those who need intensive care are all critically ill patients, but not all critically ill patients need intensive care. Special nurses are assigned to monitor the patient day and night, and sometimes the patient needs to be moved to the emergency room or intensive care unit. According to the special care plan, body temperature, pulse, respiration, blood pressure are measured regularly, the condition is closely observed, the amount of diet and excretion is recorded, basic care and daily care are carried out, turning over and massage are performed, etc. The charges for special care are relatively high, and the superiors have charging standards. Self-paying patients should consider their own ability to pay.

2. Level 2 care is marked in blue, indicating that the condition is not dangerous. It is suitable for patients in the recovery stage of critical illness with stable conditions, or elderly and frail patients who cannot take care of themselves completely and should not do much activity. For secondary care patients, rounds are required every 2 hours. During this time, if there are changes in the condition or special needs, the patient can use the call bell to call the doctor or nurse.

3. Level 3 care is ordinary care and is not marked. For mild patients at this level of care, nurses make rounds every 3 hours.
