Dynamic measurement of CA15-3 helps to detect early recurrence of stage II and stage III breast cancer patients after treatment; when CA15-3 is greater than 100U/ml, it can be considered that there is metastatic lesion.

Carbohydrate Antigen System

In the 1980s, experts used hybridoma technology to obtain the ability to recognize tumor-specific large molecular glycoprotein antigens (carbohydrate antigen, CA) and developed a monoclonal antibody recognition system. CA is a tumor cell-associated antigen. Commonly used CA series include: CA 125 (ovarian cancer-related antigen); CA 19-9 (pancreatic and intestinal cancer-related antigen); CA 15-3 (breast cancer-related antigen). CA153 is a breast cancer-related antigen and has certain value in the diagnosis and postoperative follow-up of breast cancer. CA153 is often elevated in breast cancer, but its sensitivity is low in the early stages of breast cancer, at about 60%. The positive rate of metastatic breast cancer can reach 80%. In Europe, CA153 is often used as an auxiliary diagnostic indicator for breast cancer, and is also used for postoperative follow-up to monitor tumor recurrence and metastasis. However, other tumors such as lung cancer, kidney cancer, colon cancer, pancreatic cancer, ovarian cancer, liver cancer, etc. may also increase to varying degrees. Generally, the reference value of CA15-3 is <28U/ml.

Clinical Application:

(1) CA15-3 is elevated in breast cancer patients. The sensitivity of early-stage breast cancer is 60%, and the sensitivity of late-stage breast cancer is 80%. CA15-3 is of great value in the observation of therapeutic efficacy, prognosis judgment, and diagnosis of recurrence and metastasis of breast cancer.

(2) Other malignant tumors also have a certain positive rate. Such as: lung cancer, colon cancer, pancreatic cancer, ovarian cancer, cervical cancer, primary liver cancer, etc.

(3) For non-malignant tumor diseases such as liver, gastrointestinal tract, lung, breast, and ovary, the positive rate is generally <10%. In addition, breast cancer patients with elevated CA15-3 levels develop metastasis much earlier than those with normal CA15-3 levels. According to analysis and research, there is consistency between the changes in serum CA15-3 levels in breast cancer patients and their local lymph nodes and distant metastasis. In particular, those with distant metastases have significantly increased CA15-3 expression levels and positivity rates. Therefore, CA15-3 plays a monitoring role in breast cancer metastasis. If its serum level continues to rise, chemotherapy, radiotherapy, or endocrine therapy should be started or strengthened.

If CA15-3 is elevated, further examination may be necessary to rule out the possibility of a tumor. However, the specificity of these indicators is not high and there are many influencing factors. Therefore, they are mainly used for preliminary screening of tumors in clinical practice and cannot be used to confirm the diagnosis of tumors.
