When it occurs, the patient will feel pain when bending the knee, and the knee will also become red and swollen. Pain in the knee when you bend it in the morning may be caused by rheumatism, strain, sprain or synovitis. Causes of synovitis 1. Synovitis in the elderly: Knee synovitis is often secondary to knee osteoarthritis in the elderly. It is mainly due to the mechanical and biochemical stimulation caused by cartilage degeneration and bone hyperplasia (bone spurs), which leads to synovial edema, exudation and effusion of the knee. 2. Synovitis in children: Children's synovitis usually occurs during travel and mountain climbing, or after a cold or fever. Children usually have done a lot of exercise or walked for a long time in a short period of time, which is a trigger for mountain climbing. Some children also develop the disease due to falls and sprains. 3. Excessive exercise: It is common in patients who have not exercised for a long time and suddenly take part in long-distance running sports such as mountain climbing or marathon, or go out for a long time walking, which increases the amount of exercise. The joint swelling type is mainly swelling after excessive exercise, and the pain varies in severity. Treatment of sterile synovitis: (1) Drug treatment: Currently, the main treatment drugs are non-steroidal anti-inflammatory analgesics, which are mainly divided into two categories: oral and topical. Topical medications are all anti-inflammatory analgesics and have limited therapeutic effects on chronic synovial inflammation or the acute phase of severe trauma. Oral medications have similar mechanisms of action to topical medications and can relieve discomfort such as pain. For patients with concurrent arthritis, glucosamine sulfate or hydrochloride can be used in combination. (2) Extraction of joint fluid (not recommended): The main purpose is to reduce the pressure in the joint cavity and improve symptoms such as joint swelling and pain and limited flexion and extension. It is suitable for patients with large amounts of joint effusion (sterile requirements must be strictly followed during the operation to prevent joint infection). For patients with more severe synovitis, you can also consider using an elastic bandage to apply appropriate pressure to the affected knee joint after the suction operation to slow down the production of effusion. (3) Intra-articular injection therapy: Including blocking needles, sodium hyaluronate injections, etc., are often combined with joint cavity fluid aspiration treatment. First, the joint cavity effusion is aspirated as cleanly as possible, and then the blocking needle or sodium hyaluronate injection is injected directly into the joint cavity, and then appropriate pressure is applied with an elastic bandage. (4) Eliminate the factors that may cause synovitis: For synovitis caused by acute injury, it is necessary to immobilize the joints for 2-3 weeks. At the same time, the joints should be protected from cold, exercise should be avoided in the acute phase, and walking distance should be reduced. Children with synovitis can also receive appropriate skin traction. (5) Surgical treatment: For young patients with synovitis caused by rheumatoid arthritis, ankylosing spondylitis, and villonodular synovitis, surgical resection of the synovium can be considered when necessary to avoid further damage to the articular cartilage and bone tissue. |
