1. Hip osteoarthritis

Many times, the hip pain people feel is not bone pain, but pain in the hip joint. The most common cause of hip pain in middle-aged and elderly patients is osteoarthritis. Since the hip joint (consisting of the femoral head and acetabulum) is also one of the body's heavy-loaded joints, the incidence of degenerative osteoarthritis is not low. The essence of this disease is the degeneration, cracking and shedding of cartilage on the bone surface, which leads to changes in the load stress within the joint, and then secondary bone hyperplasia (osteophyte formation) and narrowing of the joint space, which can be called "surface damage" of the bone.

2. Femoral head necrosis

Another important disease that causes hip pain is femoral head necrosis. Unlike hip osteoarthritis, this disease is caused by necrosis of bone cells under the femoral head due to lack of nutrition, and the cartilage on the surface of the joint is intact. Alcoholism and previous use of large doses of glucocorticoids are high-risk factors for femoral head necrosis.

Femoral head necrosis is not common in middle-aged and elderly patients, but it has a higher incidence rate among young and middle-aged people aged 30 to 50. Its clinical symptoms are far more severe than osteoarthritis. The pain is more severe and obvious, movement disorders appear earlier, and severe cases require crutches to walk. In the later stage, when the femoral head collapses completely, there will be a greater chance of complications such as joint deformity, dislocation or loss of function. In terms of treatment, there is currently no specific drug similar to glucosamine, and joint replacement surgery is often used in the later stages.

3. Other important diseases that cause hip pain

In addition to hip osteoarthritis and femoral head necrosis, which are relatively common and important, there are many other diseases that cause hip pain, such as ankylosing spondylitis involving the hip joint, congenital hip dysplasia, and tumors around the hip joint. However, these diseases are relatively rare and their clinical manifestations are complex, requiring the help of professional physicians to make a clear diagnosis.
